- Attachement 1 Revised SOW 9.10.15 Amendment 000001 for NIEHS2015034 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 in conjunction with FAR 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (1) This solicitation document, NIHES2015034, is issued as a Request for Quotation (RFQ) to provide continued support of Preventative Maintenance and Repair of the Mass Spectrometry Equipment at the National Institute of Environmental Health Science (NIEHS) located in Research Triangle Park, North Carolina. (2) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Regulation (FAR); HHS Acquisition Regulation (HHSAR); Federal Acquisition Circular (FAC) 2005-82 effective May 7, 2015. (3) Notice of Set-Aside, Associated NAICS Code, and Size Standard: This solicitation competed on a full and open competitive basis. The associated NAICS code is 811219; "Other Electronic and Precision Equipment Repair and Maintenance" and has a size standard of $20.5M. (4) List of Contract Line Item Number(s) and items, quantities and units of measure, (including option(s): See document titled RFQ, NIHES2015034, Preventative Maintenance and Repair of the Mass Spectrometry Equipment. (5) Description of Requirements for the Items to be Acquired: The purpose of this procurement is for the maintenance and repair service on six (6) existing pieces of equipment manufactured by the Waters Corporation. Waters Corporation is the original manufacturer of the instrumentation. The Contractor shall provide all parts, travel, labor, materials, equipment, qualified personnel, other items and non-personal services necessary to perform inspection and preventive maintenance, software maintenance, and repair services on Government owned Waters manufactured instrumentation as required at the NIEHS, Research Triangle Park, North Carolina, as defined in this Statement of Work (SOW. The Contractor shall provide: - One (1) annual mechanical and electrical preventative maintenance inspection during the contract period. - Emergency repair services on unlimited basis during the term of this contract. - All required replacement parts shall be new or remanufactured to original equipment specifications. - Software service in accordance with the manufacturer's latest established service procedures, to include telephone access to technical support for use of program software and troubleshooting of operating systems. (6) Date(s) and place(s) of performance and acceptance and FOB point: The geographical scope includes the NIEHS main campus (111 T. W. Alexander Drive, Durham, NC) and surrounding NIEHS support campuses (Keystone, repository, inhalation facility and warehouse). The Contractor shall perform service in other locations on the NIEHS campus. The contract's anticipated base period shall be from 1 October 2015 through 30 September 2016 with two (2) one (1) year option periods. (7) The provision at FAR 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (9) FAR 52.212-3, applies to this acquisition: Representation, Certifications, and Other Statements of Offerors - In accordance with provisions FAR 52.212-3, Offeror Representations and Certifications--Commercial Items, IF YOU INTEND TO SUBMIT A PROPOSAL, YOU MUST: (1) Go to the Online Representations and Certifications Application (ORCA) and complete the Representations and Certifications.
