Description
This is a Sources Sought Notice with intent to sole-source a firm fixed price one year purchase order with 2 (1) year option periods. This is not a request for quote, the NF/SG Veterans Healthcare System, Gainesville VA Medical Center, 601 SW Archer Road, Gainesville, FL 32608 is conducting Market Research to determine the availability of contractors capable of providing scheduled maintenance and emergency repair on various Steris Sterilizer equipment. The applicable NAICS code is 811219. Interested contractors are requested to submit a capability statement outlining their ability to meet or exceed this potential requirement no later than 4:30 pm (Eastern Standard Time), September 21, 2015, stating their business size, as well as a point of contact. Interested contractors must be registered in SAM. The government will use the results of this announcement to determine whether the acquisition of the above described requirement should be solicited as a small business set-aside, or whether full and open competition procedure should be utilized. THIS NOTICE IS NOT A REQUEST FOR COMPETITIVE QUOTES and no solicitation will be issued; This Sources Sought Notice is to assist the VA in identifying an appropriate acquisition strategy only. A solicitation package is not available at this time. Responses should be sent to tamela.grandberry@va,gov. No telephone responses will be accepted. SEE STATEMENT OF WORK STATEMENT OF WORK Background: The Malcom Randall VA medical Center, The Villages VA OPC, VA Tallahassee OPC, VA Jacksonville OPC, and Lake City VA Medical Center maintains an annual full service contract on all SPS Steris equipment. Objective: This Service Contract is an annual full service contract to keep all SPS Steris equipment in good operating condition. Scope of Work: Regularly scheduled visits to conduct preventive maintenance inspection at frequencies recommended by the original equipment manufacturer. Also, any corrective maintenance required to keep the units functioning at manufacturer specifications. Note- these units do not contain VA sensitive information. The contractor shall submit to the Engineering Service or the COTR, a service, report indicating what repairs, service, calibration, etc., were performed. Each service ticket report must, at minimum, document the following data legibly and in complete detail: A.Name of Contractor B.Name of FSE who performed service C.Contractor service ESR Number/Log number D.Date and time (starting and ending) equipment downtime and hours on site for service call. E.VA PO# covering the call, if outside normal working hours F.Description of problems reported by COTR/user. G.Identification of equipment to be serviced: INV, ID#, manufacturer's name, devises name, model#, Serial #, and any other manufacturer's identification#. H.Itemized description of Service performed including labor and travel, parts (with part#) and materials and circuit location of problem/corrective actions. I.Signatures: a.FSE performing service described b.VA Employee who witnessed the service described Metro Office Products This report shall be signed by the using designee attesting that the equipment is in satisfactory working order and is functioning at an acceptable level of performance. Note: If the ticket is not signed and/or submitted, payment may be withheld pending resolution. Qualification / Certifications / Safety The contractor shall obtain all necessary licenses and or permits required to perform this work. He/she shall take all reasonable precautions necessary to protects persons and property from injury or damage during the performance of this contract. He/she shall be responsible for injury to himself/herself, his/her employees, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by his/her employees fault or negligence and shall maintain personal liability and property damage insurance having coverage for a limit as required by the law of the state of Florida. Further, it is agreed that any negligence of the government, its officers, agents, servants and employees, shall not be the responsibility of the contractor hereunder with regard to any claims, loss, damage, injury and liability resulting therefrom. Vendor also note: You are responsible for providing PPE (Personal Protective Equipment) for your employees. The VA Safety Officer shall have full authority to see that the contractor obeys all safety rules and regulations to the fulfillment of this contract. Scheduling Hours of Work: All work involved shall be scheduled with the Biomedical Engineering section at 352-548-6000x2433.
