This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice.The solicitation number is W912NR-15-R-0076 and is issued as an invitation for bids (IFB), unless otherwise indicated herein.The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-84. The associated North American Industrial Classification System (NAICS) code for this procurement is 423450 with a small business size standard of 100.00 employees.This requirement is a [ Small Business ] set-aside and only qualified offerors may submit bids.The solicitation pricing on www.FedBid.com will start on the date this solicitation is posted and will end on 2015-09-15 15:00:00.0 Eastern Time or as otherwise displayed at www.FedBid.com.FOB Destination shall be New Orleans, LA 70117 The National Guard - Louisiana requires the following items, Brand Name or Equal, to the following: LI 001: TraumaFX? Patient Simulator - AirwayPlus Lifecast Pulses/Breathing (APL-PB) - Upper Body - KGS-TFX-APLPB-1 - TraumaFX? AirwayPlus Lifecast - Pulses/Breathing (APL-PB) Upper Body with Standard Starter Accessories Kit. Storage case included. The APL-PB UpperBody is a simulation of a severely injured person with a complete, realistic airway for any method of airway intervention, and breathing. The APL-PB trains responders to perform life-saving tasks such as maintaining a patient's airway, needle decompression, chest tube insertion, cricothyroidotomy, IV placement, and intraosseous infusion. The system is composed of a seamless silicone skin molded over a strong urethane and anodized aluminum core that houses all electrical and electromechanical components. The APL-PB can be paired with any TraumaFX Lower Body., 1, EA; LI 002: TraumaFX? Patient Simulator - Packable Hemostatic Trauma Trainer (HEMO) - Lower Body - KGS-TFX-HEMO-1 - Packable Hemostatic (HEMO) Trauma Trainer lower body with full left leg containing venous bleeding and single amputation with Standard Starter Accessories Kit. Storage case included. The HEMO Lower Body is a simulation of a traumatic single amputation with pulsing arterial bleeding requiring the placement of a tourniquet or direct pressure. This simulator also displays a groin injury involving a severed femoral artery that requires wound packing to achieve homeostasis, and has a lower leg crush injury with venous bleeding. The system is equipped with rugged animatronics to mimic the leg movement of a severely injured person. The system is composed of a seamless silicone skin molded over a strong urethane and anodized aluminum core that houses all electrical and electromechanical components. The HEMO can be paired with any TraumaFX Upper Body., 1, EA; LI 003: TraumaFX? Training - Basic Course - KGS-TFX-BTR-1 - KGS Train-the-Trainer user training course for up to 5 participants at a CONUS site. Training course includes all necessary training materials and leave-behinds including the Basic Course provided based on the customers purchased TraumaFX? systems on electronic media/CD. Certificate of completion and certification are given to participants upon course completion. Our Basic Training Course is scheduled to align closely with the arrival of the purchased units to the end user (this could take up to two weeks from the time the unit is delivered to a consolidation point or receiving warehouse). The customer-owned units will be used for the training. Includes T&E ONLY if scheduled at least 14 days in advance of training and is located at a CONUS site. If not scheduled at least 14 days in advance of training or OCONUS training is required, T&E will be charged as ODCs per government per diem rates., 1, EA; LI 004: Starter Kit - KGS-TFX-STK - Includes:- Makita Battery Charger ? Makita- Garment- Storage/Transit Case, 2, EA; LI 005: 1 Year basic warranty for all items, 1, EA; Solicitation and Buy Attachments ***Question Submission: Interested offerors must submit any questions concerning the solicitation at the earliest time possible to enable the Buyer to respond. Questions not received within a reasonable time prior to close of the solicitation may not be considered.*** For this solicitation, National Guard - Louisiana intends to conduct an online competitive reverse auction to be facilitated by the third-party reverse auction provider, FedBid, Inc. FedBid has developed an online, anonymous, browser based application to conduct the reverse auction. An Offeror may submit a series of pricing bids, which descend in price during the specified period of time for the aforementioned reverse auction. National Guard - Louisiana is taking this action in an effort to improve both vendor access and awareness of requests and the agency's ability to gather multiple, competed, real-time bids.All responsible Offerors that respond to this solicitation MUST submit the pricing portion of their bid using the online exchange located at www.FedBid.com.
