- Gerald R. Ford Job Corps Center, located at 110 Hall Street SE, Grand Rapids, MI is soliciting cost proposals for a licensed Optometrist to provide vision services through subcontract agreement for a facility of 200 students, ages 16-24. The minimum requirement is a Doctor of Optometry Degree and license. Services are planned to be provided on Wednesday Mornings in the practitioner's office for each new class of 3-15 new enrollees for 48 weeks of the year, exclusive of summer and winter holiday breaks. Services are to include all tests and inspections normally included in an eye exam: record medical history and complaints, note medications, perform retinoscopy, refraction, autorefactors and aberrometers, cover test, split lamp examination, glaucoma test, visual field test and dilation. Exams will conclude with a written copy of the vision prescription provided to the enrollee. Enrollee visits will provide a subjective examination of external eye and lids, eye movements and pupil reflexes, the writing of a RX to verify patient examination results, assisting enrollees selecting attractive and properly fitting frames. Eyeglass prescriptions should be filled by an approved laboratory with proper adjustment and alignment of the finished glasses to the patient's face. New eye glasses will include first quality single vision CR-9, or single vision plastic lenses with prescription up to plus or minus four diopters sphere power and up to minus three diopters of cylinder power. Higher prescription charges should be stated per lens, per diopter sphere and/or cylinder power if in additions to chart of charges. Charges should be itemized. Costs for high index lenses, tints, and scratch coating must be a part of your cost proposal. Please include costs for services such as foreign body removal and treatment, eye injuries, and infections, and the price of contact lenses. Responses to this solicitation should include a copy of your current license, qualifications brief, and names & credentials of office associates (licensed) who will/can provide treatment for Job Corps enrollees. Proposals must be received by the Purchasing Specialist no later than October 11, 2015. Your submission will be time and date stamped. Job Corps is a national training and employment program administered by the United States Department of Labor to address multiple barriers to employment faced by disadvantaged youth. This subcontract award covers December 1, 2015 through November 30, 2016. Subsequent awards are contingent on continued government funding to MINACT, Incorporated.
