Description
This is a combined synopsis/solicitation for brand name or equal commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA251-15-Q-0580 is issued as a request for quotes (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12. The North American Industry Classification System (NAICS) number is 811219. All responsible sources may submit a proposal which shall be considered by the contracting officer. To be technically acceptable an equal item must adhere to the below specifications: In accordance with the attached Statement of Work (SOW) specifications the below is requested: ETS-Lindgren Acoustic Systems Custom Double Wall Audiometric Test Booth: Booth dimensions outside: 8'8"W x 9'0"D x 8'10"H Booth dimensions inside: 6'8"W x 7'0"D x 6'6"H Floor, low profile floor system, 4 ¾" step up/slab recess Quantity: 1 FOB Destination. Installation is needed no-later-than October 30, 2015. Items will be shipped to: Wyoming Health Care Center, 5838 Metro Way Street S.W., Wyoming MI. 49519 Please see attached statement of work for more details. The following clauses and provisions apply: 52.203-99 - Prohibition on Contracting With Entities That Require Certain Internal Confidentiality Agreements (DEVIATION 2015-02) (FEB 2015). 52.204-7 - System for Award Management. The offeror shall provide its DUNS, or if applicable it's DUNS+4 number, with its offer which will be used by the contracting officer to verify that the offeror is registered in the SAM database. Lack of registration in the SAM database will make an offeror ineligible for award. Offerors and contractors may obtain information on registration via the internet at http://fedgov.dnb.com/webform, or if the offeror does not have internet access it may call Dun and Bradstreet at 1-866-705-5711. 52.212-1 - Instructions to Offerors - Commercial Items applies to this acquisition and offerors must comply with all instructions contained within it. 52.212-2 - Evaluation - Commercial Items, the Government will award a purchase order resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation is the lowest price technically acceptable; that is most advantageous to the government; price and other factors considered. 52.212-3 - Offeror Representations and Certifications - Commercial Items. A completed copy of this provision shall be submitted with the offer unless ORCA registered which must be noted in quotation. 52.212-4 - Contract Terms and Conditions - Commercial Items. 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items to include: 4, 7, 8, 25, 26, 27, 28, 30, 31, 32, 33, 36, 40, 44, and 50. 52.252-2 - Clauses Incorporated by Reference. The following additional clauses apply VAAR 852.203-70 - Commercial Advertising, 852.232-72 - Electronic Submission of Payment Requests, 852.246-70 - Guarantee and 852.246-71 - Inspection. DESCRIPTION/SPECIFICATIONS/WORK STATEMENT I.SCOPE OF WORK: The contractor shall provide materials, labor, equipment and supervision for the installation of the audio booth in room 1E112 of the Wyoming Health Care Center, 5838 Metro Way SW, Wyoming MI. 49519. The sound booth suite shall consist of a double walled booth installed by a factory certified installation team. ETS-Lindgren Acoustic Systems Custom Double Wall Audiometric Test Booth: Booth dimensions outside: 8'8"W x 9'0"D x 8'10"H Booth dimensions inside: 6'8"W x 7'0"D x 6'6"H Floor, low profile floor system, 4 ¾" step up/slab recess Construction: 1.Double walled with double swing in/out door system. 2.Acoustic wall panel construction consisting of a 16 gauge steel outlet skin, 1" gypsum, 3" non-combustible acoustic installation, and a 22 gauge perforated inner skin. 3.Heavy gauge steel welded H-joiner system between panels to maintain structural integrity and acoustic performance of the wall and roof panels. 4.Acoustic fill material between panel joints and acoustic ventilation duct liner shall be a natural cotton material consisting of a minimum 85% post-industrial recycled content. 5.Acoustic "low profile" isolated floor system with carpet, 3" thick floor with 1 5/8" vibration isolation rails, for pit recess. Doors: 1.Acoustic doors with doubled magnetic seals with heavy duty lift off hinges. 2.32" clear opening in-swing/out-swing double door system. 3.Windows in doors: 20" wide by 60" high. 4.Rubber door stops. Patient Observation Window: 1.Acoustic window for patient observation, 24" wide by 30" high (limited to booth size). Ventilation: 1.Acoustic ventilation silencer system, roof mounted supply and exhaust silencers with dust collars for a 6" flex dust connection to the buildings HVAC system. Power Outlets: 1.Two (2) recessed quad outlets beneath window, inside. 2.Two (2) recessed quad outlets beneath window, outside 3.Three (3) additional duplex outlets 4.All panels with recessed power devices to carry a UL Classification label. 5.Wyoming Health Care Center to provide one (1) 20 amp dedicated 110V electrical service to each booth with connection via flexible conduit to the provided junction box on the roof of each booth. Cable Access and Penetrations: 1.Clinical jack panel beneath window containing twelve (12) ¼" gold plated stereo phone jacks, four (4) USB connections, and one (1) 1 ½" diameter cable pass-through. 2.Conduit drop, rough in for fire alert device above inside window. 3.Conduit drop, rough in for phone/network, interior adjacent to window. 4.Conduit drop, rough in for thermostat, interior above light switch. 5.Penetrations for sprinklers, details will be provided for installation. Lighting: 1.Ceiling mounted high output / high efficiency / low heat LED lighting. 2.Dimmer switch located adjacent to interior door. Finish (Color to match existing booths here at the WHCC): 1.Powder coat paint finish with window accent trim 2.Extended roof apron trim, 8" above roof line of the booth. 3.Vertical closure panels, up to 6" air space. Warranty and Maintenance: 1.Service: Manufacture must maintain a certified service office within 200 miles of the Wyoming Health Care Center. 2.Installation and operations manual provided at no charge. 3.Pre-installation site evaluation by installers. 4.One year warranty on all parts and labor. Acoustic Test Data: 1.Manufacture must demonstrate laboratory noise reduction of NIC 68 when tested in an accredited acoustic test laboratory with the ASTM E596-96 (200) standard. 2.Test report must comply with the reporting as per the standard and indicate the booth tested is of the same general construction as the booth quoted. II.CONFORMANCE STANDARDS: All services provided under this contract must be performance in conformance with the Federal Regulations and BCVA policies and requirements. III.PREVENTIVE MAINTENANCE: Not Applicable IV.PARTS: To be supplied by vendor. V.REPORTING REQUIREMENTS: The Contractor shall be required to report to Susan Honaker or Kirk Watson, at the clinic site location in Wyoming MI. during normal business hours. This check is mandatory. When the installation is complete, the Contractor shall document services rendered on a legible field service report. The Contractor shall log out with Susan Honaker or Kirk Watson and submit the field report to the Contracting Officer Technical Representative (COR) for a signature, during normal working hours. If the COR is unavailable, a signed authorized copy of the field service report shall be sent to the COR after the work has been reviewed. VI.SAFETY REQUIREMENTS: In the performance of this contract, the Contractor shall take such safety precautions as the Contracting Officer may determine to be reasonably necessary to protect the lives and health of the occupants of the building. The Contracting Officer shall notify the Contractor of any safety issues and the action necessary to correct these issues. Such notice, when served on the Contractor or their representative at the work site shall be deemed sufficient for the corrective actions to be taken. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work, and hold the Contractor in default.
