Description

This is a SOURCES SOUGHT NOTICE ONLY - This synopsis is for informational and notification purposes only and is not to be construed as a commitment made by the Government. No contract will be awarded as a result of this Sources Sought announcement. The Department of Veterans Affairs (VA), Network Contracting Office (NCO 17), is planning to make a sole-source award for a preventative maintenance and repair agreement for a GE Aestiva 7900 anesthesia machine for the North Texas Veterans Health Care System, Dallas VA Medical Center. This requirement includes preventative maintenance, service repairs, technical support, and all required software updates for a multiple-year contract. If there are any qualified vendors of interest, please send responses to the below email address. The anticipated period of performance for this requirement is for a base term with four one-year option years with the total duration not to exceed 60 months. The corresponding North American Industry Classification System (NAICS) Code is 811219 -- Other Electronic and Precision Equipment Repair and Maintenance, with a Small Business Size Standard of $20.5 million. Interested responders should submit a capability statement not to exceed five pages that clearly addresses the following requirements: The Company has to demonstrate experience in providing maintenance services on a GE Aestiva 7900 anesthesia machine for a minimum of at least twenty-four months during the past thirty-six months. Interested responders should also show proof of certification and/or training for this specific anesthesia machine. Additional information: (1) Name of organization. (2) Organizational address(s) and telephone number. (3) Organization's point of contact and contact information including telephone number and e-mail address. (4) Current GSA contract number. (5) Current business size status for the applicable NAICS. (6) DUNS number. (7) Small Business Status {if any} (8) Total number of years of experience in providing this service. (9) Current and/or past performance reference information for requirements of similar size that includes the contract/order number, customer, and customer contact information, contract value, period of performance, prime or subcontract, and a brief description of services provided under the contract/order number. Respondents will not be notified of the results. Based on the responses received, the Contracting Officer reserves the right to determine the set-aside status. No reimbursement will be made for any cost associated with providing information in response to this request for information. All responses must be submitted by e-mail to the point of contact identified below; no other method of transmittal will be accepted. Responses to this Sources Sought Notice shall be e-mailed directly to the Contract Specialist, Nicole H. Dorsey, at nicole.dorsey@va.gov. Please include "GE Aestiva 7900 Maintenance Agreement" in the subject line of the email along with the RFQ identifier number, VA257-15-Q-1629. Responses must be received no later than 12:00 PM CST on September 18, 2015. Failure to submit all information requested on a timely basis will result in a contractor not being considered as an interested business.
