- ATTACHMENT 6 - Past Performance Survey ATTACHMENT 5 - Past Performance Survey Cover Sheet ATTACHMENT 4 - Requirements for Office Equipment ATTACHMENT 3 - Space and Business Service Requirements ATTACHMENT 2 - BEX Offsite Office Space Requirements ATTACHMENT 1 - Location of Offsite Assessment Centers This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This will be a 100% Small Business Set-aside. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. HR1025530002 is issued as a Request for Quotation (RFQ). The Government intends to acquire a commercial item using FAR Part 12 and the Simplified Acquisition Procedures set forth in FAR Part 13. The North American Industry Classification System Code [NAICS] is 531120. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-82. CLAUSES INCORPORATED BY REFERENCE FAR 52.212-1, Instructions to Offerors - Commercial, 52.212-3, Offeror Representations and Certifications - Commercial Items, FAR 52-212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The selected Offeror must submit a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items. Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commerical Items, will apply: FAR 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, and 52.232-34. The full text of a FAR clause may be accessed electronically at http://www.acqnet.gov/far. Offerors must be registered in the System for Award Management (SAM) Database prior to submission of an offer to be considered for award, pursuant to FAR 4.1102 and other applicable regulations and guidelines. Information can be found at www.sam.gov. The Offeror shall provide a quote based upon firm fixed price totals for BEX offsites. Offerors shall provide costs to complete all requirements as outlined in SOW. No travel will be required. Quote shall be submitted via email to mansfieldg@state.gov Attn: Gwendolyn Mansfield, Contracting Officer by 4:00 PM, Eastern Standard Time on Tuesday, September 22, 2015. Proposal shall be marked with RFQ HR1025530002. A cover letter signed by the company representative may accompany the quote to set forth any information that the offeror wishes to bring to the Government's attention. Quotes will be accepted via email only. Late submissions will not be considered. The burden of ensuring the Vendors' quote is received by this office, within the stated timeframe, is the responsibility of Vendor. All quotes must be valid for 30 days from the closing date for this solicitation. Communication Concerning Request for Quotation Only the Contract Administrator or the Contracting Officer may be contacted regarding this solicitation. Communication concerning this solicitation with any other Government personnel, including contractor support personnel, is prohibited unless specifically authorized elsewhere herein. If such contact occurs, the Offeror making such contact may be excluded from award consideration. RFQ Preparation Costs This Request for Quotation (RFQ) does not commit the Government to pay any costs incurred in the submission of any proposal/quote. The RFQ does not commit the Government to procure or contract for said services. The contracting officer is the only individual who can commit the Government to the expenditure of public funds in connection with this proposed procurement. Statement of Work for BEX Offsite The Contractor shall provide professional office space and equipment at a single location for the U.S. Department of State's Board of Examiners (BEX) Foreign Service Oral Assessment offsite. Office space must be located in a fully functional building that includes basic amenities such as bathrooms, water fountains or faucets, working elevators, regular janitorial service, etc. Assessment centers must be located within the downtown San Francisco area, in areas considered by local police to be safe and with a low incidence of crime. As part of its recruitment and hiring process for the Foreign Service, the U.S. Department of State's Board of Examiners for the Foreign Service (BEX) conducts oral assessments of candidates for both generalist and specialist positions in Washington, DC and San Francisco, CA. Offsite oral assessments occur in San Francisco twice annually for a period of four weeks each in Winter (January 4-29, 2016) and Spring (May 2-27, 2016). In order to conduct oral assessments at offsite locations, BEX requires certain resources, which include office space, office furnishings, computers and other equipment. The contractor is required to provide these resources as specified in the attachments. The contractor must provide to the Department a proposal that includes: • A detailed floor plan of the proposed space, including room dimensions and distances between rooms. • A list of equipment, furniture, and supplies to be provided, including computer hardware and software, Internet access, telephones, and all other requirements listed in Attachments 4 and 5. • A plan for providing other support, including names and contact information for building management. This support must be provided on-site or on-call from 6:30 a.m. to 5:00 p.m. on set-up and assessment days. The Government contemplates award of a Firm Fixed-Price purchase order or contract for this requirement. Period of Performance: One Year - September 28, 2015 through September 27, 2016 CLIN/Line Item 0001 Description: BEX Offsite Assessment Center office space 4 weeks and office furnishings (desks, chairs, etc.) Computers and equipment (copier, shredder, Printers, fax machine, etc) Internet and phone connectivity, and services Firm Fixed Price for 4 weeks (January 4-29, 2016) CLIN/Line Item 0002 Description: BEX Offsite Assessment Center office space 4 weeks and office furnishings (desks, chairs, etc.) Computers and equipment (copier, shredder, Printers, fax machine, etc) Internet and phone connectivity, and services Firm Fixed Price for 4 weeks (May 2-27, 2016) 4.0 Place of Performance Work shall be accomplished at Contractor's location and/or at the Offsite location in San Francisco. 5.0 Type of Task Order This will be a firm fixed price task order. 7.0 Security Clearance No security clearance is needed. 8.0 Performance Period The period of performance is for one year. 9.0 Safeguarding Information The Contractor and its employees shall exercise the utmost discretion in regard to all matters relating to their duties and functions. They shall not communicate to any person any information known to them by reason of their performance of services under this Task Order, which has not been made public, except in the necessary performance of their duties or upon written authorization of the Contracting Officer. All documents and records (including photographs) generated during the performance of work under this Task Order shall be for the sole use of and become the exclusive property of the U.S. Government. Furthermore, no data, database records, article, book, pamphlet, recording, broadcast, speech, television appearance, film or photograph concerning any aspect of work performed under this Task Order shall be published or disseminated through any media without the prior written authorization of the Contracting Officer. These obligations do not cease upon the expiration or termination of this Task Order. The Contractor shall include the substance of this provision in all contracts of employment and in all subcontracts hereunder. BASIS FOR AWARD Technical Evaluation Criteria The Government will make award to the responsible contractor whose offer conforms to the requirements of the performance work statement and is evaluated as being the most advantageous to the Government, cost or price and other factors considered. These criteria are listed in order of importance, with the first criteria being the most important. A. Technical/Management Approach The Government will assess the relevance, quality, and nature of the vendor descriptions or organizational experience of the same or similar scope and complexity in performing work of the same or similar nature as that of this requirement. The Government will assess the detailed floor plan of the proposed space, list of equipment, furniture, and supplies, and plan for providing other support for adherence to the Government's needs as outlined in the SOW. B. Organizational Experience The government will assess the relevance, quality, and nature of the vendor descriptions or organizational experience of the same of similar scope and complexity in performing work of same or similar nature as that of this requirement. C. Past Performance The Government will evaluate past performance information to assess the offeror's likelihood of success in performing the requirements stated in the solicitation. Currency and relevance of the information (as determined by the Government), the source of the information, content of the data and general trends in the contractor's performance may be considered. The proposal evaluation will include past performance references for the offeror and any teaming partner(s) or joint venture partner(s) that will act as a co-prime contractor. In order for the experience of teaming partners or joint ventures partners to be considered, the arrangements must be identified and company relationships fully disclosed in the proposal. Firms whose proposals are based on teaming arrangements that do not provide a signed teaming agreement(s) with the firm(s) that have been proposed will receive an unacceptable evaluation for this factor. Past performance experience of proposed subcontractors and third party providers will not be considered for this factor. To gather past performance information for evaluation, a questionnaire developed by the Department of State may be sent to the references provided in the offeror's proposals. The Department will contract only with firms that have demonstrated high levels of customer satisfaction on projects or similar size, scope and complexity. Information utilized will be obtained from the references the offerors have listed in the proposal, other customers known to the Government, consumer protection organizations, and others who may have useful and relevant information. The following five elements will be considered as a whole in evaluating an offerors' past performance on the basis of customer feedback: Quality of Product and/or Service Timeliness of Performance Price/Cost Control Business Relations Customer Satisfaction In evaluating past performance, the Government will consider the available information and give it such weight as it deems appropriate. If provided, favorable and unfavorable references for an offeror will not be averaged. One unfavorable reference, if deemed by the Government to be reliable and relevant to performance of the solicited services, may be a basis for a negative evaluation not withstanding other more positive information. Conversely, a reliable and relevant favorable reference may be given substantial weight in a positive evaluation. Firms lacking relevant past performance history will be given a neutral rating. D. Cost/Price The proposed price will be evaluated for whether it is realistic for the work to be performed, reflects a clear understanding of the requirements, and is consistent with the requirements of the solicitation. Cost/Price is not a numerically weighted factor and will receive less consideration than technical merit and past performance. The proposed cost/price will be evaluated on the basis of price reasonableness which may be done by comparison of proposed prices received in response to this RFQ with previously proposed prices under same or similar contract or by comparison of proposed prices with Independent Government Cost Estimates.
