Description
Solicitation W912P8-15-T-0103 (DD 1449) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This will be a small business set-aside procurement for health services. NAICS: 541690. Quotes are due on 22 September 2015. The solicitation documents for this procurement will be posted on Army Single Face to Industry (ASFI). You can access these files from a link located on FedBizOpps (www.fbo.gov). You will need the Adobe Acrobat Reader for.pdf files (www.adobe.com) in order to open most files posted by the New Orleans District. You are encouraged, for marketing purposes, to register as an Interested Vendor in FedBizOpps (www.fbo.gov). This can be done by clicking on the solicitation title and clicking on the button that says Register as an Interested Vendor and entering your business information. Offerors are also encouraged to post notices of prospective subcontracting opportunities on the Small Business Administration Subnet, at http://web.sba.gov/subnet. Offerors must be registered with the www.SAM.gov to receive a Government contract award. SCOPE OF WORK Contractor shall provide accurate, concise, and timely data reports and health risk assessment data processing services to the U.S. Army, Corps of Engineers, New Orleans District as specified below. Contractor shall design and provide a multiple question health survey, with space for biometrics for Blood Pressure, Lipid Profile, PSA, glucose, weight, height, hip and waist measurements, etc., referred to as a Health Risk Assessment (HRA) and the survey answer sheet which shall be a standard Scantron (shade in the circle type) or a fill in the blank form. The Contractor will be required to provide as many surveys as required; however, not more than 500 will be required each year. These HRAs shall be completed by Corps employees and the completed HRAs shall be mailed to the contractor for actions outlined below. Contractor shall collect data provided by the Corps' Occupational Health Clinic on the HRA answer sheets. Raw data from the HRA answer sheets shall then be reviewed for quality assurance and data shall be manually input into a confidential data base to provide individual personal wellness profile reports back to the Corps' Occupational Health Clinic. Within thirty (30) days after receipt of the survey answer sheets, the contractor is required to submit completed customized reports designed to inform employees of their personal health risks and detail info on how they can change or modify their unhealthy lifestyle characteristics. One overall annual group report comparing lifestyle and health characteristics of our employee population to other employers nationally is required at the end of the calendar year ending December 31, as well as, an overall report comparing first employees report of the year at start of Active For Life (AFL) program with second report, six months later, showing comparison of employees health status after the AFL program.
