- RFQ-PA-15-00021 Schedule B and RFQ Clauses Solicitation Number: RFQ-PA-15-00021 Notice Type: Combined Synopsis/Solicitation Synopsis: This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6 and 13 as supplemented with additional information included in this notice. The Solicitation RFQ-PA-15-00021 is issued as a Request for Quotation (RFQ). Award will be made using Simplified Acquisition Procedures in accordance with FAR Part 13. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Award will be made using Simplified Acquisition Procedures in accordance with FAR Part 13. Anywhere in this solicitation where the term "Bid", "Offer", or "Proposal" is used, it is hereby changed to read "Quote." Anywhere in this solicitation where the term "Bidder" or "Offeror" is used, it is hereby changed to read "Quoter." This procurement is 100 percent set aside for small business concerns and only qualified offerors may submit quotes. The associated North American Industrial Classification (NAICS) code is 312112 and the business size standard is 500 employees. Contract Type: The government anticipates awarding Firm Fixed Price base period of (12) months, plus four (4), twelve (12) month option year contract. Award will be made to the lowest price technically acceptable vendor quote which conforms to the requirements within this solicitation. In order to be considered for a government award, the firm must be registered in SAM @ www.sam.gov. Description of Services: This requirement is for lease and maintenance of Energy Star Water Cooler/Heater Filtration Units to include installation, maintenance, and removal cost as per attached Water Filtration Unit Specs. Water Filtration Unit Specs & Locations 1.Bi-annual exchange of filters. 2.No additional cost for service calls due to cooler malfunction. 3.If cooler cannot be repaired at time of service, the unit will be replaced at no additional cost. 4.tank ¿less instant hot is the preferred method for heating 5.Units must be energy star units 6.Response time for malfunctioning units 24-72 hours 7.Locations are subject to change as per attached pricing sheet EPA 1650 Arch Street Philadelphia, PA 19103 ¿(15) Free Standing Water Filtration Units Location: 1 free standing unit located in basement Fitness Center, 14 units located in kitchenette areas floor 2 through 16. ¿(1) Counter-top Water Filtration Unit Location: RA ¿s kitchenette area on the 16th floor EPA Warehouse 3900 Columbia Avenue Chichester Business Park Building 1 Linwood, PA 19061 ¿1 free standing unit located outside handicap restroom adjacent to utility sink. Emergency Operations & Conference Center 7 Chelsea Parkway Suite 707 Boothwyn, PA 19061 ¿1 free standing unit located in kitchenette area See Attachments for complete details Period of Performance: Base and four option years. Base Year: November 1, 2015 - October 31, 2016 Option Year 1: November 1, 2016 - October 31, 2017 Option Year 2: November 1, 2017 - October 31, 2018 Option Year 3: November 1, 2018 - October 31, 2019 Option Year 4: November 1, 2019 - October 31, 2020 APPLICABLE CLAUSES AND PROVISIONS The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-83. The following provisions and clauses are hereby incorporated by reference and shall apply to this solicitation and shall apply accordingly to the subsequent award. FAR 52.212-1, Instructions to Offerors - Commercial Items (Apr 2014) - The NAICS code for this procurement is 312112 and the business size standard is 500 employees. FAR 52.212-2 Evaluation ¿ Commercial Items (Oct 2014)(Tailored) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: For this acquisition award will be based on low price technically acceptable and past performance. In addition to considering past performance information provided in the vendor ¿s submission, past performance evaluations may also be based on the vendor ¿s supplied references, information obtained through federal performance tracking databases, and other information obtained by the Government from other sources. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). The Government intends to award on an all or none basis. (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. The Contracting Officer may exercise the option by written notice to the Contractor within 15 days of the required services. Delivery of added items shall continue at the same rate that like items are called for under the contract, unless the parties otherwise agree. 52.217-8 Option to Extend Services. (Nov 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. Provision EPA-K-04-101 -- Representation By Corporations Regarding A Felony Conviction Under Federal Law Or Unpaid Federal Tax Liability (Apr 2012) Instructions, Conditions and Notices to Offerors 1. Instructions for the Preparation of Quotes: The offeror shall submit one (1) electronic copy of the quotation - via email to: Alexandra Rajkowski-Reyes at: rajkowski-reyes.alexandra@epa.gov. Pricing: The offeror shall submit fixed unit monthly prices which is to include all line items and any other additional charges in the monthly cost. Please see attached Quote Sheet. System for Award Management (SAM) Requirement - By submission of an offer, the offeror acknowledges that a perspective awardee shall be registered in the System for Award Management (SAM) database prior to award, during performance and through final payment of any contract resulting from this solicitation. If the Offeror does not become registered in the SAM database in the time prescribed by the Contracting Officer, the Contracting Officer will proceed to award to the next otherwise successful registered Offeror. Registration information can be found via the internet at https://www.sam.gov or by calling 1-866-606-8220. Pricing Required for All Services and Performance Periods Offerors must quote prices for all services required during the Base Year, as well as for each ordering period, in order to be considered for award. For award purposes the Government will evaluate offers utilizing the total price for the base year and all options. Delivery of Quotations to EPA Region III Quotation Submission: The quotation due date (closing date) is at or before 4:30 pm Eastern Time, October 5, 2015. Please submit via email to: rajkowski-reyes.alexandra@epa.gov Failure to submit a complete quote by the time and date established may result in the quote not being accepted. By submitting quotations, offerors agree to comply with all terms and conditions as set forth in this RFQ, including the SOW. Quotation submissions should be E-mailed in sufficient time to arrive at the Contracting Officer ¿s desk prior to the closing date and time established in the solicitation. Vendor ¿s submitting a quote bear the responsibility to ensure that quotation packages are received by the Contracting Officer prior to the closing date and time established in the solicitation. Quoting via E-mail - due to firewalls and security protocols it is recommended that quotes be emailed at least 30 minutes prior to closing. Also please note that zipped files will not pass through our firewalls. Please send quotes via PDF, Word, or WORD PERFECT. If this is a problem please contact the undersigned immediately. Failure to submit a complete quote by the time and date established may result in the quote not being considered.
