Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA250-15-Q-0431 and is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. This is an open market competition and the associated NAICS code is 611310 and size standard is $27.5 million. The requirement is for Mental Health Intensive (MHICM) and Integrated Dual Disorder Treatment (IDDT) training and consultation. See attached statement of work description of requirements. Delivery location: All training and consultation will occur at the Chalmers P. Wylie VA Ambulatory located at 420 North James Rd., Columbus, Ohio 43219. This is destination Evaluation: will be made using the procedures under FAR 13. Offers will be evaluated on technical and past performance. In addition to filling out the price schedule, offers should provide three points of contacts, to include contact information, from three past jobs similar in size and scope. Offers should also include best expected delivery date of services. (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Offers must include the following information for evaluation: Criteria for selection: 1. Offerors must be able to provide evidence of the ability to produce fidelity review for comprehensive training for Integrated Dual Disorder Treatment (IDDT), Assertive Community Treatment (ACT) and Supported Employment (SE). 2. Offerors must provide evidence of the ability to produce Fidelity reviews on IDDT, ACT and SE. 3. Offerors should be able to provide on-site consultation as needed. The ability to provide training, consultation and fidelity reviews in person and on-site as scheduled. Undesired phone/video services. 4. Offerors will be able to provide assistance=guidance and consultation on data analysis with the collection of outcomes. 5. Offerors that provide at least 5 years of proven experience training and providing fidelity reviews to IDDT teams will be viewed more favorably. 6. Offeror must provide evidence of the ability to provide training on-site as needed and at a reasonable cost. Training will be provided for the following domains; the tenants of Integrated Dual Diagnosis Treatment (IDDT), Assertive Community Treatment (ACT) and Supported Employment (SE). Some other areas of training would consist of; motivational interventions, addictions, mental illness, case management, stages of treatment, etc. Training needs to be held in person as opposed to other methods (i.e. phone, video). 7. Offerors that provide evidence of experience working with VA programs will be viewed more favorably. 8. Past Performance Information a.Name and contact information, including an e-mail address, for three (3) clients receiving the same or similar services within the past three years. b.Completed Past Performance Surveys from the three (3) entities listed above. Completed Past Performance Surveys must be returned directly to the contracting office by the person completing the survey. It is the offeror's responsibility to ensure that the surveys are returned by references directly to the Contracting Officer in a timely manner. Technical and past performance when combined are equal in accordance with FAR 15.304, the relative importance of all other evaluation factors, when combined, when compared to price. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
