Description
THE PURPOSE OF THIS AMENDMENT IS TO CORRECT ERRORS IN THE DRAFT PERFORMANCE WORK STATEMENT References to Kamiah, ID have been replaced with Grangeville, ID. References to Lewis County, ID have been replaced with Idaho County, ID. References to estimated number of veterans residing in area covered by contract has been changed from 200 to 525. Paragraph 1 of the Draft PWS has changed. Below is the full text of the corrected paragraph 1: 1.Purpose: The Department of Veteran Affairs, Jonathan M. Wainwright Memorial VA Medical Center, Walla Walla, Washington, intends to contract with community healthcare providers in order to improve access to primary care & associated health care services for veteran beneficiaries and to furnish the timely delivery of primary care and healthcare services for veterans in their local communities. The community served under this contract is Grangeville, Idaho that lies within Idaho County, Idaho. The contractor shall provide for the continuous delivery and management of primary care and services for all assigned VA patients. Referrals for specialty care, extensive diagnostic work-ups and non-emergency hospitalization shall be made to the Jonathan M. Wainwright Memorial VA Medical Center (JMWM VAMC). There are an estimated 525 veterans residing in the area covered by this contract (Idaho County), Grangeville, Idaho area. Approximately 1% of this population is female. The most common outpatient diagnoses treated include diabetes mellitus, hypertension, arterial fibrillation and atherosclerotic heart disease. The most common Diagnostic Related Group (DRGs) for patients moving to the acute care setting are congestive heart failure, chronic obstructive pulmonary disease, artherosclerosis, angina, and cardiac arrhythmia and conduction disorders. The Veterans Administration (VA) estimates that each patient will make three visits per year to the contract primary care facility. This amount is an estimate and is to be used for information purposes only. The contractor shall provide health care providers, medical facilities, medical equipment and supplies, specified pharmaceuticals, and all administrative functions to fulfill the level of care and range of services as may be required by the VA Medical Center and in accordance with The Joint Commission or other regulatory agencies.
