Description

W-9 Self-Certification Form MTC Terms & Conditions Exhibit A Bid Sheet Request For Quote Management and Training Corporation (MTC), operator of the Tongue Point Job Corps Center, is requesting proposals for various motor vehicle/vessel supplies. Details of the solicitation are in the attached request for quote (RFQ# TPJCCMiscSupplies15-26.pdf). All bids must be completed on the attached Exhibit A Bid sheet to be considered valid. You are welcome to bid on all of the products listed; unless you are not able to quote each item. In addition to the bid, all valid bids must include a W-9 and a completed MTC vendor self-certification form (attached).
