- DENTAL PRECEDURES PRICE LIST SOLICITATION SF 603-A HEALTH RECORD This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Paper copies of this solicitation will not be available. (ii) Solicitation W912LR-15-T-0065 is issued as a Commercial Solicitation using Simplified Acquisition Procedures found in FAR Part 13. (iii). It is the contractor's responsibility to be familiar with applicable clauses and provisions. (iv)This acquisition will be 100% total small business set-aside. The NAICS code is 621210 with a small business size standard of $7,000,000.00 FSC code is Q503 - Dentistry Services. (v) The contractor shall provide all management, labor, material, equipment, certifications and supplies necessary to provide dental care services at their Personal Office, for the Puerto Rico Army National Guard Soldiers. This service will provide dental treatment, proper documentation for each Soldier's, as necessary to meet Department of the Army standards and ensure mission capability of the Puerto Rico National Guard. Evaluation factor is price that can conform with solicitation scope of work. Contractor shall posses and presents a valid professional license to practice medicine in Puerto Rico.
