- THIS IS AN AMENDMENT TO A PREVIOUSLY SYNOPSIZED SOLICITATION,issued for the purpose of re-structuring the price schedule to match the design specifications, because the price schedule did not match up to the actual number of signs being requested, as titled in the design spec, for the two different sizes (one of the sizes has two different sign titles). The revised combined synopsis-solicitation attachment no. 5 dated 9/16/15 is hereby revised again, and re-attached as Attachment no. 7 dated 9/17/15. This new attachment no. 7 shall supersede and replace the previous version which was Attachment no. 5 dated 9/16/15. The response date is not changed. Quotes are still due by 4:30pm (PT) on 9/18/15. 01 - ATTACHMENTS TO THIS SOLICITATION. The full text of the combined synopsis/solicitation is saved as a.pdf attachment which can be accessed via the link in this synopsis page where you'll be redirected to FedConnect, or by going directly to the FedConnect website at https://www.fedconnect.net. Once at the FedConnect home page, click "Search Public Opportunities Only." Next screen, under Search Criteria, change the drop-down menu selection to read "Reference Number," then type in solicitation number P15PS02151 and click "search." Next screen, click the underlined title "(JODA) National Monument Entrance Signs." Next screen is the overview screen. On the right-hand side, is the list of documents posted, showing the Standard Form 1449 (SF-1449) as Attachment no. 1, the NPS design specifications as Attachment no. 3, Amendment 0001 form SF-30 as Attachment no. 4, the Amendment 0002 form SF-30 as Attachment no. 6, and the REVISED Combined Synopsis-Solicitation as Attachment no. 7. Ignore previous Attachments no. 2 and 5. THE COMPLETE SOLICITATION PACKAGE IS COMPRISED OF ATTACHMENTS 1, 3, 4, 6, AND 7. 02 - DESCRIPTION. The National Park Service, John Day Fossil Beds National Monument (JODA) has a requirement for a vendor to manufacture/supply two different sizes of park entrance signs in accordance with the scope of work herein and the attached NPS design specifications. One of the sizes of signs specified is on Page 2.0 of the design specs, for the Sheep Rock Unit. The other size is specified on Page 3.0 of the design specs for the Painted Hills Unit and on Page 4.0 for the Clarno Unit. A total of five signs are specified, however, the quantities in the price schedule are optional because the Government has limited funds, and will purchase as many of each sign as it can with available funds. 03 - SCOPE OF WORK. (a) Signs will be constructed per the design and graphics specifications provided (see Attachment no. 3 in FedConnect), i.e., Western Red Cedar with one piece Porcelain graphic strip; (b) All signs will come with mounting hardware; and (c) Vendor shall provide final design proofs for approval by COR before manufacturing begins. 04 - DELIVERY REQUIREMENTS. Should the option for delivery as F.o.b Destination be exercised at time of award, the delivery address is as follows: John Day Fossil Beds National Monument, Attention: Jeremie Johnston, 32651 Highway 19, Kimberly, OR 97848. 05 - PRICE SCHEDULE. Refer to the price schedule in Attachment no. 7 posted in FedConnect. 06 - CLAUSES & PROVISIONS. Refer to complete list of clauses and provisions, and basis for award, in Attachment no. 7 posted in FedConnect. 07 - ANY QUESTIONS REGARDING THIS SOLICITATION MUST BE SUBMITTED IN WRITING VIA EMAIL (PHONE OR FAX QUESTIONS WILL NOT BE ACCEPTED).Send email to Phaedra_Fuller@nps.gov and reference the solicitation number in the subject line of the email, such as "P15PS02151/QUESTION." 08 - WHEN AND WHERE TO SUBMIT QUOTE: Quotations must be received at Olympic National Park no later than 4:30pm Pacific Time (PT) on Friday, September 18, 2015. Quotes may be submitted by hand-delivery, mail, or email. Telephone or Fax quotes will not be accepted. If quote is emailed, write "P15PS02151/QUOTE" in the subject line of the email. If quote is mailed or hand-delivered, the solicitation number shall be indicated on outer envelope as well as vendor's name and address. Quotations may be hand delivered to Park Headquarters, Administration Building, Offices 2 or 4 on the first floor. The address for mailing or hand-delivery is Olympic National Park, 600 East Park Avenue, Port Angeles, WA 98362. 09 - WHAT TO INCLUDE IN QUOTE: (a) SF-1449 Form (refer to Attachment no. 1), filling in business name, address, and phone number in box 17a, and filling in boxes 30a thru 30c, (b) SF-30 Amendment 0001 (refer to Attachment no. 4), filling in box 8 and 15a thru 15c, (c) SF-30 Amendment 0002 (refer to Attachment no. 6), filling in box 8 and 15a thru 15c, (d) Fill-in prices using the Price Schedule provided in Section 05 of Attachment no. 7, including a price for each line item, and (e) Provide past performance information that includes at least three client/customer references for similarly provided manufactured signs that includes a brief description of what was supplied, customer business/contact name, phone number, and email address if available. 10 - POINT OF CONTACT FOR THIS SOLICITATION is Phaedra Fuller, Contract Specialist, Olympic National Park, telephone: 360-565-3009, email: Phaedra_Fuller@nps.gov.
