Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The combined synopsis/ solicitation number is VA262-15-Q-1366. This solicitation is issued as a Request for Quote (RFQ). This solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC). This requirement will be procured as a Small Business Set-Aside under NAICS Code 339114 Dental Equipment and Supplies Manufacturing, 500 employees. The VISN 22 Network Contracting Office, Long Beach, CA, (hereafter referred to as the NCO 22) will solicit and award this request using Federal Acquisition Regulations (FAR) Part 13.03 and acquisition of Commercial Items in FAR Part 12. All questions are due by Friday September 18, 2015 at 3PM Pacific Standard Time (PST). All correspondence must be in writing via e-mail to oscar.becerril@va.gov. All quotes are due by no later than 3:00 PM PST on Monday September 21, 2015. Please provide all requested documents and schedule with pricing. NOTIFICATION TO ALL OFFERORS: Federal Acquisition Regulations require all contractors conducting business with the Government to be registered in the following database: 2) The System for Award Management (SAM)- Government-wide online database for the provision of basic information relative to contract award. After initial registration, the contractor is required to update registration data as changes occur and must re-register annually to ensure all data remains current. Noncompliance with this requirement will preclude the exercising of any option periods that may be included herein and will be cause for termination of the contract at such time noncompliance is discovered. You may accomplish this registration online at the following web site: SAM - https://www.sam.gov/portal/public/SAM/ IT IS IMPERATIVE THAT YOU COMPLETE THIS REGISTRATIONS IN ORDER TO BE CONSIDERED FOR CONTRACT AWARD. AWARD CANNOT BE MADE TO A CONTRACTOR NOT REGISTERED IN THIS DATABASE SYSTEM. The following provisions and clauses will be incorporated by reference: 52.209-6 Protecting the Government's Interest when Subcontracting with Contractors Debarred, Suspended or Proposed for Debarment 52.211-6 Brand Name or Equal 52.212-1 Instruction to Offerors Commercial Items 52.212-2 Evaluation Commercial Items: The purchase order will be awarded to the Lowest Price Technically Acceptable (LPTA). The award will be made on the basis of the lowest evaluated price of quotations meeting or exceeding the acceptability standards for the requirements/technical specifications stated above and in accordance with Attachment 1. The technical evaluation will be a determination based on information furnished by the vendor. The Government is not responsible for locating or securing any information which is not identified in the proposal. The Government reserves the right to make an award without discussions. The Government reserves the right to award on all or none basis. 52.212-3 Offerors Representations and Certifications Commercial Items An offeror shall complete only paragraph (l) of this provision if the offeror has completed the annual representations and certifications electronically at https://www.sam.gov If an offeror has not completed the annual representations and certifications electronically at the SAM website, the offeror shall complete only paragraphs (c) through (o) of this provision. 52.212-4 Contract Terms and Conditions Commercial Items 52.223-11 Ozone Depleting Substances 52.247-34 FOB Destination 52.212-5 Contract terms and Conditions Required to Implement Statutes or Executive Orders---Commercial Items applies to this acquisition.
