This is a pre-solicitation notice to search for small business vendors that can provide "Recreation/Creative Arts Therapist" services as described in the attached Statement of Work, dated September 1, 2015. This is not a solicitation and no quotes are being requested at this time. Respond back by Wednesday, September 22, 2015 to the Contracting Officer via e-mail only and state if you can provide this service along with your business name and point of contact information. The Solicitation issue date will immediately follow this pre-solicitation in order to make this award by October 1, 2015. Response date for the solicitation will be September 25, 3015. STATEMENT OF WORK, For Recreation/Creative Arts Therapist, Revised: 9/01/2015 Brief description of the scope of work - Provide Recreation and Creative Arts Therapy services to the veterans in the CLC including planning and delivering a variety of therapeutic programming for the complex veteran population as well as administrative duties as assigned by the Lead Recreation Therapist. The Recreation/Creative Arts Therapist will carry out a variety of assignments relating to Recreation and Creative Arts Therapy to the CLC residents. This will include planning and leading various therapeutic groups such as art, music, games, discussions, educational topics, as well as trips out in the community and special events (parties, music performances, holiday gatherings). Additionally, the individual will be required to work with various other departments (voluntary, food service, nursing) to coordinate various activities and special events. Contract Structure - Contractor is for 15 hours per week. Start Date - October 1, 2015 through September 30, 2016 - (Contract is for One (1) year, with possible 4 year option based on funding). Lead Recreation therapist will monitor programs including veteran response and documentation. Lunch, breaks and invoice preparation are not included in contracted hours and materials to be used will be provided by the Veterans Administration (VA): Contractor will provide all supplies and instruments. *NOTE: In the case where hours are not provided as per contract (i.e. contractor illness, needs of residents) unscheduled hours within the allotted contract can be made up, however will be subject to Lead Recreation Therapists' discretion. Hours may be required to be made up on evenings and/or weekends.
