- Continuation of the 1449 SF- 1449 form This is a notice of Government's intent to solicit, negotiate and award a firm fixed priced contract for Medical Evacuation & Insurance Services for a base plus four option years. The solicitation number is AG-3A94-S-15-0104. This requirement is not set-aside for small business. Delivery location is the United States Department of Agriculture, Food Safety and Inspection Service, Beltsville Maryland 20705. A combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information is included in this notice. Prospective offerors interested in this announcement shall provide detailed information of their capabilities and certifications to clearly meet the requirements stated above which shall be evaluated by the agency. Responses(s) may be via email or on company letterhead: include company's name, point of contact, address, phone number, DUNS number, Tax ID, business size.
