SF-18 Form COMBINED SYNOPSIS/SOLICITATION #: N00178-15-T-4646 Su bmitted by: Pamela Shelton NAI CS Code: 541380 FSC Code: B541 Anticipated Date to be published in FEDBIZOPS: 21 September 2015 Anticipated Closing Date: 22 September 2015 Contracts POC Name: Pamela Shelton Telephone#: 540-653-9429 Email Address: pamela.shelton@navy.mil Description and Code: B5 - Special Studies/Analysis This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12 and 13, as supplemented with additional information included in this notice. Synopsis/Solicitation N00178-15-T-4646 is issued as a Request for Quotation (RFQ). The synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Regulation. The Naval Surface Warfare Center, Dahlgren Division (NSWCDD), intends to procure support sample analysis for environmental projects. Please include shipping/freight cost for FOB destination Dahlgren, VA 22448-5114. Simplified Acquisition Procedures (SAP) will be utilized for award in accordance with FAR Part 12. This solicitation will result in a Firm-Fixed Price purchase order with a base period of 12 months and four (12) month options. Split-sampling and blind-sampling will be performed by NSWCDD as required by Federal, State, and local statutes. The NSWCDD reserves the right to send these samples to other contractors or to Commonwealth of Virginia laboratories to maintain QA/QC standards. Emergency Analysis: An expedited testing charge will be established for a 5-day turnaround at 1.5 times the quoted amount and a 3-day turnaround at 1.7 times the quoted amount provided by the contractor. Sampling Analysis: The approximate number of samples is dependent on the task required. The quantity identified in Attachment 1, Table 1, is approximate and should only be used for the price estimate. The type of analysis conducted is summarized and represents an annual price for sampling. Field duplicate samples will be submitted with "blinded" identification to the laboratory. Additional volumes of these samples will be provided as necessary. Miscellaneous analysis includes any additional sampling analysis tasks not specified but which may be required by NSWCDD in the future. Sampling Pricing: The contractor shall provide a unit price for each test shall include the cost of providing transportation when required, laboratory analysis, reporting, and sample containers. The contractor shall include a parameter list indicating the laboratory analytical methods, detection limits and references for the methods to be used in the analysis for each of the parameters listed in Attachment 1, Table 1. Subcontractors: The contractor shall identify the use of, and percentage of use of, subcontractor(s) with their proposal. In addition, the contractor shall include proof that the proposed subcontractor(s) have complied with all required licenses and permits. The contractor shall notify the Contracting Officer and the TPOC of any decisions to add or change any subcontractors previously identified within 10 calendar days prior to the addition or change. The contractor shall provide proof of compliance regarding licenses and permits prior to the addition or change of subcontractors. All responsible sources may submit a capability statement, proposal, or quotation which shall be considered by the agency. In order to be eligible for award, firms must be registered in the System for Award Management (SAM). Quote is due 22 September 2015 no later than 04:00 p.m. EST with an anticipated award date by 30 September 2015.
