- THIS IS A SOURCES SOUGHT NOTICE ONLY - DO NOT SUBMIT PROPOSALS. The purpose of this Sources Sought announcement is to identify sources capable of providing support for this effort. The information received will be used to determine if the requirement can be set aside for small business. Responses from small businesses are highly encouraged. Based on responses to this notice, the Government may consider a small business set-aside for this requirement. The Department of Veteran Affairs, Network Contracting Office 19 has a requirement for a VentricularAssist Device Technical Support in the Salt Lake City (SLC) area or the Salt Lake City (SLC) Valley area with the following qualifications: Provide qualified Physician Assistants or Nurse Practitioners to perform 24 hour VAD coverage. Qualification requires a master's degree from an accredited Physician Assistant or Nurse Practitioner Program and a minimum 1-2 years as a Physician Assistant or Nurse Practitioner with LVAD, Transplant-Heart & Lung experience. Must have a current Utah (or BNE compact state) license with BNE recognition as an Advanced Practice Nurse and prescriptive authority by the BNE and BME. Must be a Registered Acute Care Nurse Practitioner with a BCLS and ACLS Certification, and a Current Advanced Cardiac Life Support Certification. Services include but are not limited to the following: "Administrative Preparation for VAD Evaluations i.e. order tests for VAD evaluation, follow up on results, and compile all patient information for presentation to VAD selection committee. "Pre-VAD Clinic/inpatient visits to include; pulling chart information and lab/procedure results in preparation for clinic, support physicians and surgeons, Intro to VAD for patient and family members, evaluation of post-VAD support structure, initiation of post-VAD discharge plan/s, obtain necessary consents. "Patient transport to include; make all transport arrangements with EMS, prepping patient for transport, accompany patient to VA, report to bedside nurse, exchange equipment if necessary, arrange transport back to contractor facility if a return to the OR is necessary. "Daily Rounds at the VA, download, compile, and organize patient information, perform rounds with physician/surgeon at the contractor's facility and the VA Hospital, Follow up on post-rounding instructions, coordinate discharge of patient from the contractor's facility and the VA hospital. "VAD Education to include; evaluate and educate inpatients and outpatients for VAD therapy, Series of at least 5 sessions with patient and caregiver prior to discharge, Provide pre and post implant education to families and caregivers, provide long-term patient management, escort patient/s home from the VA (travel expenses for escort to be billed separate), average travel for out of region patients is 2 days; in-region 1 day, provide education for primary physician, local ER, local EMS, referring cardiologist, and any neighbors or friends who will be supporting the patient. "Provide Post-VAD clinic/Outpatient Visits to include seeing patient/s weekly for 4 weeks post-surgery, then every two weeks for another month, and then monthly thereafter until transplant or death, prepare patient charts and information, support physicians and surgeons, perform dressing changes as needed, perform continued education as needed, perform medication review, device checks and interrogation, and perform scheduling. "Provide 24/7 on-call coverage for patient issues related to VAD support. This 24/7 coverage is also available to VA nursing staff, referring physicians, caregivers, and patients with VAD-related questions. "Provide OR Procedure Support to include; Assist surgeon in OR or Cath Lab for centrimag implant as needed; Provide VAD device support during implant, catheterizations, ECHO, and other procedures. "Provide support for VAD patients in the VA Emergency Department (ED), in the event that a VAD patient is seen in the VA ED a coordinator will come in to support the ED staff in the care of the patient. "Input National data entry required by Joint Commission Standard DSPM.4, updated monthly per patient from the time of implant until death or transplant. "Provide initial training for SICU and CVMU - 3 hours annually per unit, provide recertification for SICU and CVMU - 2 hours annually per unit. "Additional Administration Duties include; Perform as needed administration functions - average one day annually per patient (i.e. EMS, Letters/Memos, device inventory and ordering, education manuals, etc.). Travel Costs: One of the service components itemized is to escort patients home and perform training for their caregivers. This cost is to be itemized separately and identified. VAD coordinator travel time is two 10 hour days. Transportation, lodging, and meals are to be itemized with receipt copies provided. Interested business concerns, large or small, should review this Sources Sought Announcement and submit information for consideration. The anticipated NAICS Code is 622310, Specialty (except Psychiatric and Substance Abuse) Hospitals with a small business size standard of $38.5M. The anticipated contract will contain a 12 month base period plus four (4) 12-month option periods. Interested organizations are required to identify their size standards in accordance with the Small Business Administration. The Government requests that no proprietary or confidential business data be submitted in a response to this notice. However, responses that indicate the information therein is proprietary will be properly safeguarded for Government use only. Interested business concerns, large or small, should review this Sources Sought Announcement and submit information for consideration electronically (via email) to Michael Vela, Contract Specialist at michael.vela@va.gov by 4:00 PM Mountain Time, 10/02/2015. This notice does not obligate the Government to award a contract or otherwise pay for the information provided in the response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After review of the responses received, pre-solicitation and solicitation notices may be published in Federal Business Opportunities or VA GSA. However, responses to this notice will not be considered adequate responses to a solicitation. The solicitation release date is pending. The Government intends to negotiate a fixed-price contract.
