- RFQ HSFLGL-15-Q00441 This is a combined synopsis/solicitation for commercial items prepared in accordance with format Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation HSFLGL-15-Q-00441 is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 05-83. This procurement is a 100% Small Business set-aside and the associated NAICS code 541519 with Size Standard $24.5 M. The Federal Law Enforcement Training Centers (FLETC) has a requirement for Installation of AVID Interplay Bundle Media Server System done in 3 phases. Period of Performance: Completion within 7 months after award. In accordance with the attached Statement of Work: CLIN 0001 - Task 1 - Design Phase 1 EA CLIN 0002 - Task 2 - Installation Phase 1 EA CLIN 0003 - Task 3 -Quality Assurance Phase 1 EA Submission of Information Offers are to provide the following: a) Pricing - The offeror shall submit total price inclusive of all 3 phases. Task 1 - Design Phase Task 2 - Installation Phase Task 3 -Quality Assurance Phase b) Technical - The Offeror shall submit the following: 1) Resume: The offeror shall provide a resume that clearly demonstrates the experience and certification of the Minimum Qualification requirements outlined in the Statement of Work (SOW), Task 7.0; certified television and broadcast engineer with 10 years of experience to include Avid installation experience. 2) Quality Assurance Plan: The offeror shall provide a brief narrative that describes the proposed strategy to meet requirements of SOW, Task 3.3 to include the 24 hours response requirement during the "bake in'" period. 3) References: The offeror shall submit three (3) references of relevant past performance to the requirement to include contract numbers, description of services provided, and point of contact information. "Relevant" is defined as work that is the same or very similar to the work being identified in the SOW. Failure to provide all above elements will be determined as non-responsive offer. See attachment for complete details. Offeror must be registered in The System for Award Management (SAM) database before an award can be made to them. If the offeror is not registered in the SAM, it may do so through The System for Award Management (SAM) website at http://www.sam.gov. Quotes shall be submitted electronically via email Gretchen.Lovell@fletc.dhs.gov and are due no later than September 25, 2015, 4:00 PM EST.
