- Statement of work to be completed Vendor set up form 502 SF 1413 Davis Bacon Act wage requirement from www.wdol.gov SF 542 REQUEST FOR PROPOSAL Project Title: Concrete walkway and workspace A. General a. Type of Solicitations: This is a subcontracting opportunity for concrete b. Location of Agency/Office: Cleveland Job Corps Center Location: 13421 Coit Road Cleveland, Ohio 44110 c. Site Visit Date: September 9, 2015 at 10am d. Closing date of submission: September 15, 2015 by 4:00pm B. Period of Performance: September-October C. Statement of Work Project Title: Concrete walkway and workspace Background: Students walking on campus have created a dirt pathway around the H-Dormitory going towards the center of campus Bricklaying trade needs additional cement area to store supplies Scope of Work: The contractor will excavate in two locations The first location is around the perimeter of H-Dorm (included drawing of building), it will start and attach to the current walkway and wrap around the building attaching the walkway that runs through the campus. Originating and terminus points of the new sidewalk will be flush with the existing sidewalks The area to be excavated is 168 feet long by 6 feet wide and 8 inches deep. The base will be filled with 4" of #57 limestone and covered with 4" of concrete (approximately 12.5 cubic yards of concrete). The concrete should be flush with the surrounding grass. The second location is behind building B, classroom B2, Bricklaying. This will be a cement pad poured next to an existing pad. The new poured pad will be 39'4"x30'. This will require the same excavation of 8 inches deep with a base of # 57 limestone covered with concrete of 4" to 6". This concrete pad will need to be the same height as the existing pad. Limestone base for both the sidewalk and pad must be packed prior to pouring of the concrete. The contractor is required to verify all measurements. Any errors will be the responsibility of the contractor to complete. D. Requirements a. Applicable Licenses b. Certificate of Liability Insurance c. Vendor to include W-9 and DUNS# in proposal d. Total Labor b. Material c. Other d. Total Price F. Evaluation Criteria To be considered for award, a proposal must comply in all material respects with the invitation for proposal. Award will be made to that responsible contractor whose proposal, conforming to the invitation, is most advantageous to Serrato Corporation, considering only price and the price-related factors included in the invitation.
