Description

The Department of Veterans Affairs (DVA), VA Caribbean Healthcare System (VACHS), Network Contracting Office SAO East 8, Admin Building #440 Suite 456 10 Casia Street San Juan, PR 00921-3201 intent to solicit offers and award one firm-fixed price contract on a competitive basis requesting the procurement of Automated Sedimentation Rate (ESR) Analyzers for the VA Caribbean Healthcare System Pathology and Laboratory Service in San Juan P.R. and the Outpatient Clinics of Ponce and Mayaguez Applicable North American Industry Classification System (NAICS) code number 423490 and Size Standard is 100. This is a competitive Request for Proposal (RFQ) using FAR Part 13 and it will be posted on FEDBIZOPPS, Lowest Price Technically Acceptable (LPTA) source selection process, CASCADING- from SDVOSB up to all SB. Award will be made to a responsible offeror on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors; only were adequate/sufficient competition at a given tier is received. The solicitation documents, all incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-27. The qualified Offeror must provide services to provide the labor, equipment and materials to perform preventive maintenance to the ESR Automated Analyzers witch are intended for the erythrocyte sedimentation rate measurement. The Erythrocyte Sedimentation Rate (ESR) is a non-specific test that alerts physicians to the possible presence of disease or abnormality in the body. The ESR is determined by measuring, in millimeters per hour, the distance red cells have fallen after separating from the plasma in a vertical sed-rate pipette. This measurement reflects changes in plasma proteins which accompany acute and chronic infections, inflammation, malignancy, multiple myeloma, macroglobulinemia and hyperfibrinoginemia. The ESR is used primarily to follow the progress of inflammatory diseases such as rheumatoid arthritis during treatment.. QUESTIONS: shall be submitted in writing via e-mail attention to the Contract Specialist no later than 7 days after the solicitation is posted at Fedbizopps (FBO) web site: http://www.fedbizopps.gov/. VERBAL OR TELEPHONE INQUIRIES WON'T BE ACCEPTED. All responses will be answered and incorporated in to a written amendment to the solicitation, for the benefit of all interested offeror's. Solicitation No. VA248-15-Q-1091 electronic request for Proposal package is available to the public as Request for Quotation(attached below). Faxed proposals will not be accepted nor e-mails. Since this is a Combine Synopsis/Solicitation Notice, Offerors will be able to see below attached Solicitation. In case the RFQ due date is extended, all interested offeror's shall be responsible to monitor the changes posted at FBO web site, prior the RFQ due date. Each offeror shall acknowledge receipt of each amendment, submitting each amendment attention to the CO at VA Caribbean Healthcare System (VACHS), Network Contracting Office SAO East 8, Admin Building #440 Suite 456 10 Casia Street San Juan, PR 00921-3201 by the RFQ due date. The contract resulting from this procurement will be awarded to the responsive and responsible offeror whose offer, conforming to the solicitation requirements will be most advantageous to the Government, in terms of price standpoint, technical factors and past performance. Offer shall include a fixed firm price for the services shown in the Schedule of Price Section of the solicitation, or offer will be unacceptable to the Government based on the solicitation terms and conditions. There will be no public bid/offer opening meeting on the solicitation due date. The Government reserves the right to award without discussions; therefore, the offeror shall submit the most favorable terms and technical information with their offer. All interested offeror's shall be registered in the System for Award Management (SAM) database before submit their offer. Contractors are responsible for the accuracy of information added to SAM. To be registered, you must go to https://www.sam.gov/sam/. Since this is going to be set aside SDVOSB, all interested vendor must be registered on VETBIZ (http://www.vetbiz.gov/) and have the seal of approval. Current contract information may be re-quested and disclosed under Freedom of Information Act (FOIA), in accordance with 38 Code of Federal Regulations (CFR) 1.553(b).
