Description

This is not a solicitation for proposals. The purpose of this notice is to conduct market research regarding the availability and capability of all qualified sources to perform Speech Language Pathology (SLP) services for the Northern Arizona VA Health Care System (NAVHCS), 500 North Hwy 89, Prescott, Arizona 86313. Responses to the information requested will assist the Government in determining the acquisition strategy. SAO West NCO 18 is conducting market research to determine the existence of potential small business concerns (e.g., HUBZone, 8(a), SDVOSB, Woman Owned, etc.) with the capability to fulfill this requirement. Description of Services: The contractor shall provide speech language pathology services including evaluation and assessment to: (a)aid the patient in regaining or maintaining the ability to communicate at his/her optimum level with non-verbal strategies that may include gesturing, sign language, reading and writing, communication boards, computer programs and other assistive devices; and verbal communication strategies that are divided into four categories including voice, language, speech and prosody. (b)aid patients with swallowing dysfunction to include instructing and educating the patient and/or nursing staff with swallowing techniques as indicated and recommending the most appropriate method for feeding; and recommendations to Radiology with assistance by SLP to perform modified barium and fluoroscopy when needed. (c)Conduct assessments in the SLP office, radiology, resident's bedroom or dining room, home setting, or for offsite cognitive group therapy sessions. (d)make recommendations for appropriate support groups or agencies for personal, emotional or miscellaneous patient needs. Personnel assigned by the Contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. All licenses held by the personnel working on this contract shall be full and unrestricted licenses. The qualifications of such personnel shall also be subject to review by the VA Chief of Staff and approval by the VA Facility Director. Each person assigned to work under this contract shall be licensed by certified by ASHA and licensed by the State of Arizona. All information shall be submitted in Word or PDF format, not to exceed 10 pages, including all attachments. Each submittal shall also include the following business information: a. DUNS number b. Organization Name c. Organization Address d. Point of Contact (including name, title, addresses, telephone number, fax number and Email address) e. Business size f. Socio-economic status ( IF SDVOSB or VOSB - provide SDVOSB or VOSB CVE verification) g. Brief overview of the firm's capabilities and past experience with projects similar in scope to this project. Firms shall identify whether or not they are a small business or other than small business in accordance with the size standard ($11.0M) associated with NAICS code 621111. If another NAICS code may be applicable, please identify the NAICS in your capability statement. All information must be received no later than October 5, 2015.
