COMBINED SYNOPSIS/SOICITATION FOR "BRAND NAME" OR "EQUAL" Synthes Screw Removal Set This is a combined synopsis/solicitation for commercial items (supplies) prepared in accordance with the format at FAR Subpart 12.6, as supplemented with additional information included in this notice, and is being conducted pursuant to FAR Part Subpart 12.6 and FAR Part 13. The synopsis/solicitation constitutes a RFQ and incorporates provisions and clauses in effect through Federal Acquisition Circular No: 2005-81 and Department of Veteran Affairs, Acquisition Regulation Circular 2008.02. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The NAICS Code is 339113 with a corresponding size standard of 500 employees. The Department of Veteran Affairs,Wilmington, Pennsylvania, has a requirement for: Clin 0001: Screw Removal Set, Stock Number, 01.240.001, Quantity 1 each ADDITIONAL DOCUMENTS NEEDED W/THE QUOTE: If offering a product other than the "Brand Name" the vendors are required to provide descriptive literature in sufficient detail to determine if the items quoted are technically acceptable in terms of "Brand Name" or "equal" to the Synthes Screw Removal Set IAW FAR 52.211-6, please provide the following if bidding an "or equal," product: A.Manufacturer's name B.Manufacturer's Part Number C. Description to include salient physical, functional, and or performance characteristics The following provisions apply to this acquisition FAR 52-212-1 Instructions to Offerers - Commercial Items (Apr 2014) This provision is incorporated by reference, and the addendum below is modified to read as follows: To assure timely and equitable evaluation of the quotes, the offeror must follow the instructions contained herein. The quote must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The response shallconsist of two (2) sections; Price & Technical Capability. FAR 52.212-2 Evaluation - Commercial Items (Oct 2014) This provision is incorporated by reference, and paragraph (a) is modified to read as follows: (a) This is a competitive source selection in which the Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price, and other factors considered. The Government intends to make award to the lowest priced, technically acceptable offerer. The Government intends to award without discussions with respective offerors. The Government, however, reserves the right to conduct discussions if deemed in its best interest. The following listed are evaluation factors that shall be used in the evaluation process: 1. Technical Capability: Technical Acceptability is achieved when the offeror provides proof that the products meet the minimum standards of the Items Description, the stated Delivery Date, and Quality). *All offerers shall submit technical description of the products being offered in sufficient detail in addition to the price proposal, to evaluate compliance with the requirements in this solicitation (b) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror, shall result in a binding contract without further action by either party.
