Description

1.This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Solicitation number VA246-15-Q-0483 (See Attached Solicitation) is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-80 corrections. North American Industrial Classification Code (NAICS) 325412, Pharmaceutical Preparation Manufacturing, is applicable. 2.This solicitation is Set-Aside 100% for Small Business (SB). 3.The source selection procedures to be used will be lowest price technically acceptable. 4.The Department of Veterans Affairs (VA), Network Contracting Office 6 (NCO-6), Durham VA Medical Center (VAMC) requires radiopharmaceuticals with specialized application for treatment to cancer patients in accordance with Public Law 104-262 and 38 USC 8153, as described in the Section B.4, Statement of Work (SOW) in the solicitation. 5.The Government anticipates award of a firm fixed price contract. The Government anticipates awarding one requirements contract with a 12 month period of performance base and 3 option years. 6.Delivery destination is: Durham VAMC, 508 Fulton Street, Durham, North Carolina 27705-3897 7.Proposals that do not contain requested instructions, licenses, certifications and applicable supply partnering agreements as detailed in Section E, Addendum to FAR Part 52.212-1, Instructions to Offerors of this solicitation, will be considered not technically acceptable and non-responsive for a contract award for this requirement. 8.Questions: any questions about this solicitation must be sent via email to Mr. Olin D. Newsome Jr. at olin.newsome@va.gov no later than 1:00 PM Eastern time September 28, 2015. 9.Quotes must be received no later than 1:00 PM Eastern Time on September 29, 2015. 10.The successful company must mandatorily be registered in the (System for Award Management) at the web site: www.sam.gov (SAM).
