Description
COMBINED SYNOPSIS/SOLICIATION ‘COMBO’:Asbestos Abatement and Carpet Installation, extension of solicitation updated. Attachment 6 Bldg 4251 Diagram, information updated. MODIFICATION TO SOW and ATTACHMENT 6 Questions and Answers in response to FA5000-15-T-0241 on FBO.gov. SUBJECT: FA5000-15-T-0241: MODIFICATION TO SOW and ATTACHMENT 6 1. The following information has been added to the Statement of Work: 5. Description of work and special Instructions: 5.1 Existing Conditions 5.1.1 Unit will remove all furniture from office prior to start of work. The contractor will identify, coordinate and communicate area where work is to begin NLT 14 days prior to start of work. 5.1.2 Abate and remove and dispose of existing carpet, carpet pad, mastic, vinyl asbestos tile as necessary to complete the work described in this document. *The area is estimated to be 37,000 square feet. *See Attachment 6 for the building diagram has been updated to reflect the tile/carpet area. 5.1.3 Remove and dispose of existing debris / adhesive residue 5.1.4 Provide a clean smooth, true level plane and surface preparation for the install of carpet. 5.1.5 Protect all existing furnishings, equipment and finishes that are not scheduled for removal. Patch and repaint areas damaged during abatement and removal process. Return all existing finishes to pre-construction conditions. 5.2 Finishes 5.2.1 Install commercial grade, high traffic carpet in a dark gray or dark blue color. 5.2.2 Provide and install coverbase/ wall base board molding to match existing in the area. 5.2.3 Provide adhesives for flooring and base accessories as recommended by the manufacturer. Also comply with local indoor air quality and hazardous material procurement and disposal standards. 5.2.4 All work to be inspected and accepted by Government Official/ Representative upon completion. 2. Attachment 6 has been modified to reflect the tiled areas, the rest are carpet. 3. The quotes will now be due by 1600 (4 pm) on 25 Sept. 2015 Alaska Standard Time. Please resubmit a quote if the changes affect your submission. Questions and Answers in response to FA5000-15-T-0241 on FBO.gov. Q: Will there be a site visit? A: The solicitation posted on 18 Sept. 2015 states that there will not be a site visit under section (xv). Q: Is there any information regarding the type of carpet they are requesting? A: The description available under section 5 of the Statement of Work states: 5.2.1. Install commercial grade, high traffic carpet. Clarification has been provided in the color choice: carpet dark gray or dark blue color. Q: Do you have better dimensions for the area that needs asbestos abatement and carpeting than what is provided in the solicitation? A: There is a diagram Attachment 6) that shows the rooms that require abatement and carpet, and one area that only needs new carpet. The estimated square feet of 27,000 is now increased to 37,000 SQ FT.
