- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. REGIONAL HEALTH CONTRACTING OFFICE - ATLANTIC, McDonald Army Health Center, Fort Eustis, VA intends to solicit, negotiate, and award a firm fix contract. This is a combined synopsis/solicitation for Cost Per Test Services at the Laboratory Department, McDonald Army Health Center (MAHC) Joint-Base Langley-Eustis, VA prepared in accordance with the format in FAR 13.5 and Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the solicitation only. This requirement is being solicited as Unrestricted. This resultant contract will be a Firm-Fixed Price type contract. This solicitation number W91YTZ-15-T-0110 is a request for proposal (RFP). Request for Proposals are being requested and a written solicitation will not be issued. Period of Performance is 01 October 2015 through 30 September 2019. The applicable North American Standard Industry Classification System (NAICS) Code 334516 with a size standard of 500 employees. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-81. Interested persons may identify their interest and capability to respond to the requirement and submit proposals. All proposals received after the date of publication of this synopsis will be considered by the Government. Submit proposals via email ONLY to Anette Smith at anette.smith5.civ@mail.mil no later than 25 September 2015 @ 2:00 pm EST. Questions concerning this acquisition may be address to anette.smith5.civ@mail.mil no later than 25 September 2015, 11:00 AM EST. No telephonic request will be honored. Clauses and provisions throughout this solicitation can be viewed by accessing website www.farsite.hill.af.mil and http://farsite.hill.af.mil/vffara.htm. All offerors are cautioned that if selected for award, they must be registered with the System for Award Management (SAM) program. The SAM can be accessed via the internet at https://www.sam.gov/portal/public/SAM/. Confirmation of SAM registration will be validated prior to awarding a contract. Vendors shall include a completed copy of FAR provision 52.212-3 Alt 1, Offeror Representations and Certifications with their offer. Offerors shall include a confirmation of receipt of any amendments to this solicitation. Failure to submit a confirmation could result in the offer being determined non- responsive. Clause 52.212-4 Contract Terms and Conditions-Commercial items, applies to this acquisition to include any applicable addenda.
