- Description
- 1. Description of Services: The vendor shall submit the results of lab test for single and multiple births to Pediatrics, Womack Army Medical Center within 7 days following the readings. Performance is from October 1, 2015 through September 30, 2016. 2. Description and Specifications: a.. Womack Army Medical Center (WAMC) staff will collect 6 blood samples from newborns at WAMC, dry the specimens and mail the specimens to the Contractor, and fill out a NC Department of Health and Human Services Division Form that contains the following information: Newborn's Medical Record Number, Newborn's First and Last Name, Newborn's birth date, Birth-time, Race, Sex, Birth weight in grams, Specimen date, Specimen time, Mother's First and Last name, Mother's Maiden Name, Mother's Social Security Number, Mother's Mailing Address, Hospital Specimen submitter, Federal Tax Identification number, Physical Name and Federal tax Identification Number. b. Collection of Specimens by WAMC staff will occur with 24-48 hours following birth of the newborn and be mailed to the State Laboratory by the 3rd day. C. The Contractor shall perform the following tests on the blood samples within 1 day of receipt of samples: -Congenital Adrenal Hyperplasia (CAH) -Galactosemia (GAL) -Congenital Hypothyroidism (Thyroid) -Sickle Cell (Hemoglobin) -Phenylketonuria (Amino Acid Profile) -Acylcarnitine Profile d. There are no charges for hearing test information- the State of North Carolina collects this information for demographic purposes only. e. If known Phenylketonurias need follow up tests. There shall be no additional charge for follow up testing. f. For those specimens collected in which the paper is damaged or rubbed up; the test will be repeated. WAMC will submit recollection specimens with no additional charges for services g. The contractor shall submit the results of those tests to WAMC within 7 days following the readings. Womack Army Medical Center Department of Pediatrics ATTN: Ms. Vivian Taylor Fort Bragg, North Carolina 28310 h.In cases of abnormal test results of the laboratory screening of the blood samples, the Contractor shall provide a report WAMC requesting additional blood samples, Enclosure 5. WAMC will collect the repeat specimens and mail or fax to the Contractor. There shall be no additional charge for services. i.By the 2nd calendar day following the last day of each month, the Contractor shall mail out a report to WAMC containing the following listed information: Name of patient, birth date, medical record number and the number of tests performed for all of those patients receiving tests during the preceding month. j.This information should match the monthly invoice for which the Contractor is seeking payment. This information will be utilized by the WAMC staff to validate services received for the preceding month.
