- W-9 Self-Certification Form MTC Terms and Conditions DBA Wage Determination Construction Bid Package This is a subcontracting opportunity with Management and Training Corporation (MTC), operator of the Tongue Point Job Corps Center. Tongue Point Job Corps Center is requesting proposals from contractors providing Painting Services. Details of the solicitation are in the attached Construction Bid Package (CBP: TPJCC Painting Bldgs. 1, 3, 6, & 19.pdf.) Please accept this as your invitation to attend our pre-bid meeting for this project at Tongue Point Job Corps Center, located at 37573 Old US Highway 30, Astoria, OR 97103. The meeting will be conducted on Wednesday, October 07, 2015 at 3:00pm (PST), with bids due on Friday October 16, 2015 at 12:00pm (PST). Please see the attached Construction Bid Package (CBP) for details. Please pay close attention to page 2 of the Construction Bid Package Instructions to Bidder. In addition to the bid sheet, all valid bids must include 3 (three) references, DBA Worker Classification/Rates, a W-9, and completed MTC vendor self-certification form (attached). Bidders must self-certify under the NAICS code listed in the CBP. (NAICS codes can be found at http://www.census.gov/eos/www/naics/) This solicitation is listed under NAICS code: 238320 - Painting and Wall Covering Contractors. Please note: A site-inspection will be required by the bidder or their appointed company representative for the bid to be considered valid. The best opportunity for this inspection will be during the pre-bid meeting at the time stated above.
