- FA8126-15-R-0005-0001 The response time for this solicitation has been extended to 2 October 2015 at 2:00pm Central Standard Time. All vendors submitting a proposal must acknowledge Amendment 0001 by signing it. All vendors who previously submitted a proposal may resubmit a new proposal. Any vendor who did not submit a proposal by the previous response time, may submit a proposal by the amended response time. No late proposals will be accepted in accordance with FAR 15.208. All vendors submitting a proposal are reminded to thoroughly read the entire proposal and specifically 52.212-1 Addendum and 52.212-2 Addendum to ensure a complete and responsive proposal is submitted. Vendors must address all technical and price factors. Vendors who fail to submit a complete proposal may be considered non-responsive and subsequently ineligible for award. Vendors are also reminded to ensure their pricing is accurate, as in mathematically correct when multiplying the unit price by the unit to calculate the extended price for each Contract Line Item Number. Vendors should ensure the commas and periods are in the correct place for each unit price and extended price. All offerors shall complete only paragraph (b) and (j) of 52.212-3 if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM). If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete paragraphs (c) through (p) of 52.212-3. All Offerors must also complete 252.209-7992. This is a 100% Small Business Set Aside procurement for the acquisition of AutoCrib brand name items. The requirement is for one basic year and 2 one-year options. The NAICS code for this requirement is 332999 with a size standard of 750 employees. To be eligible for award, the offeror must be registered in the System for Award Management (www.sam.gov) with completed and correct representations and certifications.
