Statement of Work Rental of Portable Boiler Alexandria VA Health Care System, Pineville, LA 502-16-1-5023-0001 Scope -The Alexandria Veterans Affairs Health Care System, located at 2495 Shreveport Hwy, Pineville, LA 71360 requires the rental of a portable boiler rated a minimum of 900 HP (Horsepower) at 30,000 LBS/H (Pounds of Steam per Hour) to supplement current boiler capacity and provide alternate source of steam and heat generation for the period 01 November 2015 - 31 March 2016. Objective "Boiler to be less than 10 years old "Provide an 900 HP Portable Boiler with all annsulary equipment. "Boiler and D/A Equipment to be on one trailer "Provide 24 hour response time to service calls. "Boiler shall be able to run on Natural Gas or #2 Fuel Oil "Boiler to meet VA Low NOX emmissions standards Tasks Provide the supervision, manpower, tools, materials, equipment and certifications to install Portable Boiler and connect to existing Boiler Plant connections in accordance with manufacturers guidelines and ASME Boiler Code at the Alexandria VA Health Care System, 2495 Shreveport Highway, Pineville, LA. "Provide one 900 hp enclosed trailer mounted portable Boiler "Provide labor, freight and material to install/pickup - 900 hp trailer mounted portable Boiler at VA Medical Center - Pineville La "Installation will require trailer to be leveled by contractor "Provide all necessary connection work to connect to VA Boiler Plant "Unit must have D/A tank with duplex pumps "Unit must have duplex softeners "Unit must have blow-down tank with automatic tempering valve "Provide means of chemical treatment of boiler feedwater "Boiler blowdown to be piped to nearest drain "Installation to be complete and boiler functioning within 1 week of delivery "Provide a feedwater booster pump with 1.25 inch connections to provide adequate feedwater to boiler. "Electrical connection to boiler feed water pump will be supplied by VA if necessary "Provide interior and exterior lighting for safe operation "Provide steps with handrails to boiler deck for safe operation and routine inspections "Start up boiler and check for proper operation "Provide operator training for VA Boiler Plant employees "Provide temporary security fencing around boiler equipment "Provide field service for boiler, 24 hour response time, for the rental period. "Boiler to have Boiler Inspection documention from State Fire Marshal (state of orgin and Louisiana) and National Board Boiler Inspector. "Boiler to be delivered with in 10 days of award of contract. General Requirements "All work shall meet all required codes and standards. "Work shall be done on site of Alexandria VA Health Care System, 2495 Shreveport Highway Delivery All personnel in the contractors services will have certified or proper training on the tasks they perform. The Project Engineer or COR will be informed in the following situations: "Before the start of any contract work. "After arriving on site but before any work is started. "To complete all contract work in the time specified. "As soon as it is known that the work will take longer than expected. "At completion of work prior to leaving the facility. "To report any incomplete aspects of the work or issues that could not be corrected during the visit. "To provide a written plan for corrective action for any work that cannot be completely within contract specified time. "Contractor Payment Application will be provided within 14 business days of the actual work being completed. Proper Lock-out/Tag-out procedures will be used when appropriate. All work will be completed in accordance within manufacturers recommended guidelines All required contract works completed will be approved and accepted by Project Engineer or COR prior to accepting and approving contractor application for payment. Security and General Requirements All on-site work should be completed between the hours of 8am and 4:30pm CST, Monday through Friday excluding holidays unless otherwise listed in the tasks section or other working hours are approved by the COR. Contractor will obtain an access badge from the VA Police Department or from Facility Engineering Department while performing work on-site for their self, their employees and all subcontractors under their authority. Contractor will contact Project Engineer or COR whenever work is being performed in any VA facility to determine if an escort is needed. Project Engineer or COR will arrange for an escort if needed. Security and privacy requirements vary depending on the equipment to be serviced and the service being performed. Contractor personnel may be required to complete any or all of the following but not limited to: "Online training. "Read, sign, and comply with 'Contractor Rules of Behavior'. "Submit to finger printing and background investigation. "Provide network security information as requested for any equipment used in the VA facilities. "The extend of these requirements will be determined by Project Engineer or COR using current facility Information Technology and Privacy rules in cooperation with experts in those departments. Parking will be limited to established parking lots. Contractors and personnel are not allowed on the grass area unless specifically authorized by the Resident Engineer. U.S. District Court Violation Notices will be issued for parking violations. The contractor and all employees will comply with all applicable Alexandria VA Health Care System fire, safety, and traffic regulations. The contractor is to field verify existing conditions. All work is to be performed within all OSHA, EPA, ASME, VA, and Federal, State and local codes and regulations. No roadway, drainage ditch, pedestrian walkway, or any other service, utility, or access to any of the above is to be blocked or rendered unusable for any length of time during this project unless previously approved by the Resident Engineer. All work, materials, storage of materials and equipment, repair procedures and unforeseen problems or circumstances are to be coordinated with the Resident Engineer. Work on this project will not begin until authorized by the Contracting Officer. The title Project Engineer or COR shall refer to the Contracting Officer's Technical Representative. The C&A requirements do not apply, and that a Security Accreditation Package is not required Place of Performance "Work will be performed at Building 14, Boiler Plant, of the Alexandria VA Health Care System, 2495 Shreveport Highway Pineville, LA, or in the immediate vicinity as required by this Scope of Work. "Equipment or Government property may be removed from the facilities with prior permission from Project Engineer or COR and the Department Director.
