Amount To be considered for award, offerors must submit a price for line items 1, 2, 3, 4, 5 and 6 for the base year and all four option years. One award will be made for line items 1, 2, 3, 4, 5 and 6 for the base years and all four options. All unit-of-use packaging identified in the Schedule of Supplies, (i.e., 30s), must contain a child proof closure (safety cap) and a bottle with a minimum required size of 100cc volume. The safety cap must not exceed the diameter of the bottle. The bottle must have a cylindrical body and minimum of 5.75" circumference. The bottle must fit a label with a dimension of 5.25" x 2". The 0.5" additional space is required to keep the bar code exposed and/or allow for the display of the expiration date and lot number. Glass bottles are not acceptable. Line items 1, 2, 3, 4, 5 and 6 are for unit-of-use package sizes or for bulk package sizes. Offeror must submit a price for either unit-of-use or bulk package sizes for the base year only, and unit-of-use for the option years for all line items. For the purpose of evaluating offers, $0.6165 (VA awarded repackaging fee) per bottle would be the repackaging cost to the Government during the base year of contract performance for a contract awarded under this solicitation, and will be included in the calculation of the lowest total aggregate price for evaluation of award. To be considered for award, an offeror must submit a price for line items 1, 2, 3, 4, 5 and 6 for the base year and all four option years. Award will be made in the aggregate for line items 1, 2, 3, 4, 5 and 6. The total aggregate price will be computed by multiplying the estimated quantity for each line item by the unit price offered inclusive of repackaging cost and summing the total prices of all line items. Offered unit prices shall not exceed two decimal places.
