- Synopsis: Please note that this is NOT a request for quotes or proposals. The Government is seeking information for market research purposes only. The Department of Veterans Affairs, VISN 3, Network Contracting Office 3 (10N3NCO) is currently conducting a market survey for firms capable of providing the following service: Performance Work Statement (PWS) Title: Automated IT Patient Discharge and Referral Services 1.GENERAL: This is a non-personnel services contract to provide Automated IT Discharge Services. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government. 2.DESCRIPTION OF SERVICES: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform IT Discharge Services as defined in this Performance Work Statement except for those items specified as government furnished property and services. The contractor shall perform to the standards in this contract. 3.BACKGROUND: Information Technology Discharge (IT Discharge) refers to advanced automated discharge planning and referral services that utilize information technology solutions. This case management tool was explored in support of VHA Social Work Systems Redesign endeavors in order improve the effectiveness and efficiency of VA discharges to post-acute care settings. Automated discharge referral services commonly utilized in the community are widely noted as a case management best practice. This patient centered case management tool if implemented within VHA has the potential to transition Veterans from VA acute care to non-VA post-acute care settings more efficiently than current practice. Automated discharge and referral planning tools expedite consumer driven post-acute placements resulting in improved care coordination. 4.OBJECTIVES: a.Utilize Information Technology to automate the referral/discharge process from Medical Center in-patient settings to post-acute care facilities in the community. b.Decrease the time required of VA clinical staff to process referrals to post-acute facilities and increase response time from post-acute care facilities to VA on receipt of referrals. c.Enable real-time understanding of post-acute care bed availability in the community. d.Decrease the Medical Center's average length of stay (LOS) for in-patient Veterans awaiting placement in community based post-acute care facilities. e.Decrease the Medical Center's diversion and denial of bed days of care. f.Strengthen HIPAA compliance through the use of encrypted communication to post-acute care facilities. 5.SCOPE OF WORK: Implement IT Discharge technology to automate the discharge and referral process and enable rapid communication among VA and non-VA case managers. The contractor must demonstrate they have the ability to establish a comprehensive network of post-acute care facilities subscribers in the Medical Centers catchment area. In addition, contractors must offer the option of coordinating referrals to post-acute care facilities not in their subscriber network. It's important that referrals are not restricted to subscribers only and that alternative options such as but not limited to virtual fax be available. The contractor must provide "start" up training on site at the Medical Center and develop reference training materials. The contractor must collaborate with VA to develop performance measures to evaluate the effectiveness of the implementation of the discharge referral services to demonstrate metrics such as but not limited to decreased length of stay, cost savings, as well as track and report progress on the metrics identified. These progress reports shall be provided on a monthly basis. The contractor will be expected to participate in monthly meetings with VA to review progress and performance. At the end of the performance period, the contractor will develop a final written report. This report should provide an executive summary of the service provided, challenges and lessons learned, as well as the benefits and outcomes of services with demonstrated performance measures. The contractor shall prepare and conduct an executive power point briefing highlighting key findings based on the report. This briefing may be conducted virtually. The contractor must comply with the relevant HIPAA requirements in the managing of Veterans Personally Identifiable Information (PII) and Protected Health Information (PHI). The contractor shall provide the ability to customize the user interface and templates needed to accomplish referrals to post-acute care facilities. The contractor must provide comprehensive "startup" training to VA staff who will utilize this service, as well as ongoing technical support throughout the period of contracted services. 6.PERIOD OF PERFORMANCE: The period of performance shall be for one (1) Base Year of 12 months from the date of award, with 4 additional year options. 7.GENERAL INFORMATION: a.Quality Control (QC): The contractor, as part of their proposal, shall develop an effective quality control program to ensure services are performed in accordance with this PWS. The contractor shall develop, implement and maintain procedures to identify, prevent, and ensure non-recurrence of defective services. Additionally, the contractor will be evaluated by internal quality assurance measures as they relate to contract terms. The contractor shall receive the contracting officer's acceptance in writing of any proposed change to his QC system. b.Quality Assurance: The government shall evaluate the contractor's performance under this contract in accordance with the Quality Assurance Surveillance Plan (QASP). This plan is primarily focused on what the Government must do to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s). c.Recognized Holidays: The contractor is not required to perform services on Federal Holidays. New Year's DayLabor Day Martin Luther King Jr.'s BirthdayColumbus Day President's DayVeteran's Day Memorial DayThanksgiving Day Independence DayChristmas Day d.Hours of Operation: The contractor is responsible for conducting business, between the hours of 8am to 8pm, Monday thru Sunday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. For other than firm fixed price contracts, the contractor will not be reimbursed when the government facility is closed for the above reasons. The Contractor must at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when the Government facility is not closed for the above reasons. When hiring personnel, the Contractor shall keep in mind that the stability and continuity of the workforce are essential. e.Place of Performance: The work to be performed under this contract will be performed at contractor facilities. All training is to be provided on-site at the Northport VAMC. f.Type of Contract: The government will award a non-personnel services contract, firm fixed-price (FFP). g.Physical Security: The contractor shall be responsible for safeguarding all government equipment, information and property provided for contractor use. h.Post Award Conference/Periodic Progress Meetings: The Contractor agrees to attend any post award conference convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation Subpart 42.5. The contracting officer, Contracting Officers Representative (COR), and other Government personnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings the Contracting Officer will apprise the contractor of how the Government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the Government. i.Contracting Officer Representative (COR): The COR will be identified by separate letter. The COR monitors all technical aspects of the contract and assists in contract administration. The COR is authorized to perform the following functions: assure that the Contractor performs the technical requirements of the contract; perform inspections necessary in connection with contract performance; maintain written and oral communications with the Contractor concerning technical aspects of the contract; issue written interpretations of technical requirements, including Government drawings, designs, specifications; monitor Contractor's performance and notifies both the Contracting Officer and Contractor of any deficiencies; coordinate availability of government furnished property, and provide site entry of Contractor personnel. A letter of designation issued to the COR, a copy of which is sent to the Contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting order. j.Contractor Travel: The Government shall not approve any travel expenses. k.Other Direct Costs: Direct costs shall be included in the contractor's quote. The Government agrees only to pay the firm fixed-price agreed upon in the contract. l.Data Rights: The Government has unlimited rights to all documents/material produced under this contract. All documents and materials, to include the source codes of any software, produced under this contract shall be Government owned and are the property of the Government with all rights and privileges of ownership/copyright belonging exclusively to the Government. These documents and materials may not be used or sold by the contractor without written permission from the Contracting Officer. All materials supplied to the Government shall be the sole property of the Government and may not be used for any other purpose. This right does not abrogate any other Government rights. m.Technical Support: The contractor shall provide ongoing support to include toll free customer service phone line and e-mail hotline to resolve technical issues. Issues shall be resolved within a 24 hour period. n.Orientation and Training: The contractor shall conduct on-site initial orientation and training to implement the system and shall be available for training support thereafter via phone, e-mail and/or web-based tutorials. Orientation and training shall be conducted within 30 days of contract award. Staff to be trained will include, but may not be limited to Social Workers, Nurse Case Managers, Clinical Applications Coordinators, and IT. o.Bid Proposals: Contractors shall itemize bid proposals and include a schedule of values. Contractor must demonstrate the ability to establish a network of post- acute care facilities that subscribe to their service within the Medical Center's catchment area. In addition, contractors must offer the option of coordinating referrals to post-acute care facilities not in their subscriber network. Responses to this notice must be submitted in writing via email and must be received not later than Thursday, October 8, 2015, 4:00pm EST. No telephone inquiries will be accepted. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, HUB Zone, 8(a), small business or large business). Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities for which you have provided services to, proof of applicable certification. DUNS number, address, point of contact and social-economic category (ex: SBVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business).
