Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures, specifically FAR Subpart 13.5 - Simplified Procedures for Certain Commercial Items; and as supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-83, dated September 03, 2015. Solicitation VA262-15-Q-1410 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) contract with a base year and up to four (4) option years. This procurement will be issued as a Total Small Business Set-Aside under NAICS Code 221310 with size standard of $27.5 million. Offeror(s) must be registered/certified on the SBA website (SBA DSBS Website) as a small business under this NAICS code to be considered for award. The VA Southern Nevada Healthcare System (VASNHS or Government) has a requirement for water treatment services for the domestic potable water system. The services provided shall include, but not limited to complete service oriented water treatment, sampling, and test program. The Contractor shall furnish all labor, materials, supplies, equipment and qualified personnel to perform the requested services. The Contractor shall also comply with all laws, policies, and/or regulations as required by local, state, and Federal agencies. See attached SF 1449 Solicitation for statement of work (SOW), inventory equipment list, and other pertinent information. Place of Performance: VA Southern Nevada Healthcare System, located at 6900 North Pecos Road, North Las Vegas, NV 89086 Site Visit: A site visit is scheduled for October 7, 2015 at 9:00 AM Pacific Standard Time (PST) at VA Southern Nevada Healthcare System, located at 6900 North Pecos Road, North Las Vegas, NV 89086. Offeror(s) will meet at the Energy Plant, located on the east side and meet with Tamara Feist-Hatfield, Contracting Officer as she will be hosting the site visit. Attendance is NOT mandatory, but highly encouraged so offeror(s) are able to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent the information is reasonably obtainable. All offeror(s) MUST RSVP for this site visit and limited to two (2) attendees per offeror(s). Please RSVP with Tue M. Tran via email no later than October 5, 2015 - 12:00 PM PST, ensure to reference solicitation # VA262-15-Q-1410 within subject line of the email. Prospective offeror(s) are responsible for arranging and paying for their own travel, lodging, meals and all other expenses incurred in conjunction with attendance at this conference and any subsequent building visit. No offeror(s) shall be given preferential treatment as a result of their attendance or failure to attend the site visit. Note: Any verbal information provided during the walkthrough will not be binding on the Government. Please submit all questions in writing via email to Tue M. Provisions 52.212-1, Instructions to Offerors - Commercial and 52.212-2, Evaluation - Commercial Items are applicable to this acquisition. Furthermore, Provision 52.212-2 is utilizing the Lowest Price Technically Acceptable (LPTA) on a "PASS/FAIL" evaluation process and award will be made to the offeror who conforms to the solicitation and represents best value to the Government. Evaluation Criteria: Each offer received will be evaluated by the following criterion. Each factor shall be evaluated on a "PASS" or "FAIL" basis. Submitting no Past Performance will not be deemed a fail. Quote must include all the requested information below or the offeror will be considered non-responsive and will not be considered for award. An offeror must receive a "PASS" on each factor to be considered Technically Acceptable. Quote to include technical information shall be limited to no more than 12 pages. 1.Factor 1 - Certifications (PASS/FAIL): Offeror must provide the D-1 license of all operators who will be performing on this contract. D-1 license must be current and active. 2.Factor 2 - Technical Capability (PASS/FAIL): Be able to illustrate no less than five (5) years of recent experience providing domestic water treatment services of the same magnitude as this requirement, including quality assurance. Documentation must demonstrate at least five (5) years' experience performing the requirements specified in the statement of work (SOW), with recent performance occurring within the last three (3) years. Offeror shall provide an overview of the water treatment program and the following information: a)Customer Name b)Address c)Contact Telephone d)Contact Email e)Service/Product Provided f)Service Period g)Contract Amount (annually and total) 3.Factor 3 - Technical Approach (PASS/FAIL): Offeror must communicate their approach to satisfying the requirement as specified in the SOW. Offeror shall not simply restate the content within the SOW, but detail their specific approach. 4.Factor 4 - Price: All offers shall be sorted by price (lowest to highest) then each offer will be evaluated for technical acceptability. The evaluation will stop once three (3) technically acceptable offers are evaluated. Award will be made to the lowest priced offer of those three (3). Submitting Quote: Offeror(s) may submit their quote on company letterhead and shall include the completed pricing forms, along with name, address, telephone number of the offeror, firm's DUNS#, total price and a completed copy of FAR Clause 52.212-3 Offerors Representations and Certifications - Commercial Items or a copy of offeror's annual offeror representations and certifications (ORCA) document from www.sam.gov as required by FAR Clause 52.212-3 Offerors Representations and Certifications - Commercial Items. As an alternative, Offeror(s) may use the attached SF 1449 for quote submission. Submit all question(s) to Tue M. Tran at tue.tran@va.gov no later than October 8, 2015, 2:00 PM PST. Ensure to reference solicitation # VA262-15-Q-1410 within subject line of the email. All questions and answers will be posted to this solicitation via an amendment no later than October 9, 2015.
