Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.303, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The simplified acquisition procedures of FAR Part 13 will be used in conjunction with commercial items procedures of FAR Part 12. Solicitation # RFP VA256-15-R-1046. This solicitation is issued as a request for proposal (RFP). This is a total set-aside for Service-Disabled Veteran-Owned Small Businesses only. NAICS code is 811219, size standard is $20.5 MILLION. Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83, effective July 2, 2015. A site-visit will be held October 6, 2015, at 10am Central Time. All interested vendors need to meet at: John L. McClellan Memorial Veterans Hospital 4300 W. 7th Street Little Rock, AR 72205 Ground Floor - Room GD-114H Any interested vendors shall contact Timothy Pighee via email to confirm attendance. POC: Timothy Pighee, Contract Specialist Phone: 501-257-1028Email: Timothy.Pighee@va.gov REPAIRS TO YORK CHILLERS The Contractor shall furnish all necessary labor, parts, transportation and equipment to provide an annual inspection and certification, and to maintain manufacturer's specifications on 486 government-owned patient lifts located at John L. McClellan Memorial Veterans Hospital in Little Rock and North Little Rock, AR in accordance with all terms, conditions, provisions, schedules discussed below. Model: Horcher Unilifts C-100-200-2 for localized lifts Horcher Unilifts C-100-200-11gate system lifts for transporting All work shall be done by a factory/manufacturer (Horcher) certified technician with at least 3 years of experience on patient lifts. (See Corporate Project Experience Form) Service shall consist of preventive maintenance inspection(s) (PMI), cleaning, adjusting, and replacing parts to maintain the equipment within original specifications. The Contractor shall make thorough inspection of the equipment at approximate six month intervals.
