- Department Of Veterans Affairs Robley Rex VA Medical Center, Louisville KY Requirements for A/E Services Project 603-16-602 Replace Dental Gas and Vacuum DESCRIPTION: This is a Request for information for the following services "Architectural and Engineering Services to replace dental gas and vacuum, Project# 603-16-602" at the Robley Rex VA Medical Center, 800 Zorn Ave, Louisville KY. THIS ANNOUNCEMENT IS NOT A REQUEST FOR PROPOSAL; NO SOLICITATION WILL BE ISSUED UNTIL AFTER AN EVALUATION HAS BEEN MADE ON ALL THE PROVIDED SF330's. Interested firms should submit their current SF330 to joseph.cowan@va.gov The SF330s are due on November 2nd 2015 at 2:00 PM Eastern Time. Interested A/E firms shall submit an electronic SF330 in Adobe Acrobat pdf to joseph.cowan@va.gov. Scope of work: A.Provide all A/E services necessary for the design and installation of replacement Dental Gas and Vacuum components including head units, control systems, piping and other components that serve the hospital. This contract includes all planning, phasing, design, construction services, commissioning coordination and project closeout documents required to support the installation of these systems located in the Robley Rex VAMC in Louisville, Kentucky. These systems shall be complete, coordinated, and technically correct. They shall be fully integrated with the existing monitoring and control system. In addition, these systems shall be safe, easily accessible for repairs and maintenance, energy efficient, and in accordance with Department of Veterans Affairs Design Criteria and Standards and all applicable codes found in the technical information library at the VA website http://www.cfm.va.gov/til/index.asp. The A/E is fully responsible for researching and documenting existing conditions and will be given full access during normal business hours to equipment locations and VA drawing files; access to surgical areas will be by appointment only during non-use hours. The A/E is also to explore various design solutions and construction phasing to allow the VA to evaluate the impact on the surgical department and risk to patients before deciding on the project approach. Design shall be completed within 180 days of notice to proceed. At the completion of the project the contractor will provide drawings in AutoCAD 2013 (or newer) format and specifications in Microsoft Word and PDF formats. The contractor will provide to the VA four CDs with all drawings and specs and four sets of drawings at completion of design. B.A Government Estimate for the construction phase of this will be provided under this contract. C.Construction phase A/E services will be provided under this contract with a minimum of 6 site visits. D.Commissioning services will be provided under separate contract. The A/E will coordinate with the VA selected commissioning agent. E.As-Built Drawings a)The construction contractor's as-built drawings shall be reviewed on a regular basis and revisions made to the appropriate AutoCAD files. All variations or changes shall be shown in the same general detail as used in the contract drawings. b)The A/E shall deliver approved completed sets of as-built drawings to the COR within 15 calendar days after each completed phase and after final acceptance of the project by the COR (Final drawings will be submitted on E size vellum and in electronic format, Auto CAD 2013 (or newer) and PDF on CD or DVD. c)Follow established drawing standards. in the technical information library at the VA website http://www.cfm.va.gov/til/index.asp. All objects and associated text shall be drawn in model space with similar objects on appropriately named layers. Only the drawing border and associated text may be drawn in paper space. d)An AutoCAD file shall be delivered for each paper drawing developed for the project. This file shall be purged of all extraneous references and all external references and blocks shall be bound to the file. e)The A/E shall update the VA's architectural drawings to reflect any changes as a result of this project. Drawings will be submitted on E size vellum and in electronic format, Auto CAD 2013 (or newer) and PDF on CD or DVD. f)The A/E shall update the VA's electrical drawings to reflect any changes as a result of this project. Drawings will be submitted on E size vellum, shop drawings will be replaced in kind, and in electronic format, Auto CAD 2013 (or newer) and PDF on CD or DVD. g)The A/E shall update the VA's mechanical drawings to reflect any changes as a result of this project. Drawings will be submitted on E size vellum, shop drawings will be replaced in kind and in electronic format, Auto CAD 2013 (or newer) and PDF on CD or DVD The submitted SF330s will be evaluated on the following criteria: (1)Professional qualifications necessary for satisfactory performance of required services. (2)Specialized experience and technical competence in the type of work required, including, where appropriate, experience in energy conservation, pollution prevention, waste reduction, and the use of recovered materials. (3)Capacity to accomplish the work in the required time. (4)Past performance on contracts with Government agencies and private industry in terms of cost control, quality of work, and compliance with performance schedules. (5)Location in the general geographical area of the project and knowledge of the locality of the project; provided, that application of this criterion leaves an appropriate number of qualified firms, given the nature and size of the project. (6)Reputation and standing of the firm and its principal officials with respect to professional performance, general management, and cooperativeness. (7)Record of significant claims against the firm because of improper or incomplete architectural and engineering services. (8)Specific experience and qualifications of personnel proposed for assignment to the project and their record of working together as a team. NAICS code 514310 is applicable to this procurement. The Small Business size standard is $7M. FAR Clause 52.236-2 ( c ), Design Within Funding Limitation will be applicable to this procurement action. Design limitation costs will be provided to the most highly rated AE firm selected to complete the design. All offerors shall be further advised that in accordance with VAAR 836.606-73 the total cost of the architect or engineer services contracted for must not exceed 6 percent of the estimated cost of the construction project plus any fees for related services and activities. This requirement is 100% set-aside for Veteran Owned Small Businesses (VOSB) in accordance with 38 U.S.C., section 8127(d). To be eligible to submit an offer or receive an award all offers must be viewable and listed in the Vendor Information Pages (VIP) at www.vetbiz.gov. Only verified VOSB are eligible to submit an offer or receive an award that is set-aside for VOSB in accordance with 38 CFR Part 74. Contractors that are not verified and viewable in vetbiz at the time of submission of offer or at the time of award shall be determined as "to not be an interested party and its proposal will not be considered further nor will be considered for award." This requirement is being procured in accordance with The Brooks Act of 1972, Federal Acquisition Regulation 36.6 (FAR) and VA Acquisition Regulation 836.6 (VAAR). GEOGRAPHICAL LOCALITY The A/E shall be geographically located within 300 miles driving distance of the Robley Rex VA Medical Center in Louisville KY. This distance will be determined by utilizing Google driving directions at (https://www.google.com/maps).
