- Custom Surgical Packs WORK STATEMENT 1. PURPOSE Three VISN 18 facilities located in Tucson, AZ, Phoenix, AZ, and Albuquerque, NM require custom surgical packs in order to provide surgical services to Veteran patients. Custom surgical packs are comprised of various medical supplies for specific areas, e.g., Catheterization Lab, Operating Rooms, etc., within each facility. The contract arising from this solicitation is intended to provide surgical packs to the three (3) facilities until such time as a regional contract can be awarded by the Western States Network Consortium (WSNC) Contract staff. The surgical packs shall be subject to change by bilateral contract modification as needed for each facility. 2. SCOPE The Contractor shall provide all labor, materials, transportation, equipment and supervision, in compliance with federal, state and local regulations, to provide Custom Sterile Surgical Packs for VISN 18. The contract period is anticipated to be from approximately October 1, 2015 through September 30, 2017 (one 1-year base period, plus one 1-year option period). The contract is for the procurement and distribution of the schedule items. In addition the contractor shall provide custom surgical packs in the configurations identified in the contract (attachments A, B and C) to the Southern Arizona VA Health Care System (SAVAHCS) (Attachment A), the Phoenix VA Health Care System (PVAHCS) (Attachment B), and the New Mexico VA Health Care System (NMVAHCS) (Attachment C). Each facility has defined surgical pack configurations to meet the needs of specific areas within the facility. Quantities have been estimated based on past usage, and are subject to fluctuation based on the surgical needs of each facility. The contractor shall be capable of providing the estimated quantities in the timeframes required by the contract to ensure that surgical procedure schedules are not negatively impacted.
