Description

Important Date: Please submit final quote via email to brooke.hansen@va.gov by 5:00pm Eastern Time on Monday, October 5, 2015. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is VA251-16-Q-0001and is issued as a Request for Quotes. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-69. The associated North American Industrial Classification System (NAICS) code for this procurement is 325413 with a small business size standard of 500 employees. Please submit all offers to Brooke.Hansen@va.gov before Monday, October 5, 2015 at 5:00pm Eastern Time. FOB Destination shall be Ann Arbor, MI 48105. This will be evaluated based on lowest price technically acceptable. The VHA NCO 11 - 506 - Ann Arbor requires the following items, exact match, to the following: See attached for detailed line items. Period of delivery shall be: 30 days NOTICE TO OFFERORS: Instructions for submitting quotations under this request for quote must be followed. Failure to provide all information to aid in the evaluation may be considered non-responsive. Offers that are non-responsive may be excluded from further evaluation and rejected without further notification to the offeror. In addition to written price quotes, offers are instructed to provide: Submit quotations via email at or before the time specified in the solicitation. Email quotes are acceptable and can be sent to brooke.hansen@va.gov. At a minimum the contractor shall provide the following information on their quote: a) Point of contact name, telephone and E-mail address. b) DUNS Number New Equipment ONLY; NO remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. Quote MUST be good for 90 calendar days after close of solicitation. This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34. The full text of the referenced FAR clauses may be accessed electronically at https://www.acquisition.gov/far/. FOB Destination CONUS (Continental US). No partial shipments unless otherwise specified. New equipment ONLY. NO remanufactured products. Quotations are to be submitted as all or none. No late quotations will be accepted. All questions must be emailed to Brooke.Hansen@va.gov no later than 5:00pm Eastern Time on Friday, October 2, 2015. No phone call inquiries will be accepted. Award will be made to the responsible Offeror, whose offer conforming to the solicitation, will be the most advantageous to Veterans Affairs on the basis of price, technical capability and delivery schedule.
