W9 form Brief description of business ID#: EX041516 FIRE PROTECTION SERVICE Adams and Associates, Inc., contracting for the U.S. Department of Labor for the operation of the Exeter Job Corps Academy - a 200 student residential training facility located at 162 Main Street, Exeter, RI, is currently soliciting written bids for fire protection service. All quotes should be submitted in writing to the Purchasing Department by Friday, October 30, 2015 by 5:00 p.m. Quotes received after the deadline and/or without all requested documentation will not be considered. Vendor is to return written bid on vendor letterhead, vendor paperwork, and all licenses for doing business in the State of Rhode Island, all employee licenses required for performing work in the state of Rhode Island, Certificate of Liability insurance and list of references if no past experience with Center. Vendor must refer to Scope of Work Identification number on submitted bid. Pricing is to be quoted for the period of December 1, 2015 - November 30, 2016, one year terms subject to change. Submitted bid will show at minimum breakdown as follows: •· Cost per hour - non emergency •· Cost per hour - emergency •· Printed name and Signature of authorized person submitting bid Bids, vendor questionnaire, W9, and all licensures needed to do business may be emailed to: burgess.annette@jobcorps.org ; mailed to: Exeter Job Corps, Attn: Annette Burgess, Purchasing Department, 162 Main Street, Exeter, RI 02822 or faxed to (401) 268-6080. If your quote deviates from specifications in any way, such exceptions must be noted on bid sheet. If unable to quote, please indicate 'No Bid' and return bid package to us. Adams and Associates, Inc. reserves the right to reject any bids and select the bid deemed most responsible. Bidders should be aware of bid requirements for federally funded projects and act accordingly. Bidders are placed on notice that this is a competitive bid process and cost is an important fact. Overall, low bid does not constitute bid award. Exeter Job Corps Academy is a government facility and entitled to GSA pricing. Payment terms are Net 30 days from date of invoice. All question regarding the Invitation to Bid and/or scope of work must be submitted in writing to: burgess.annette@jobcorps.org. No phone calls will be accepted. Employee time cards/payroll must be submitted for each service incident over $2,500 per Service Contract Act. SCOPE OF WORK FIRE PROTECTION SERVICE REFERENCE: SCOPE OF WORK ID#: EX041516 The vendor shall provide to Rhode Island Exeter Job Corps Academy which consists of a dormitory with 200 beds, construction building, academic building, two commercial kitchens, and wellness center, fire protection services as follows: Vendor is to provide inspections, testing of fire alarm systems, emergency lighting systems and sprinkler systems quarterly for 4 buildings Annual test of all backflow preventers with results given to the Maintenance Manager upon completion of test (4) All fire extinguishers must be inspected annually and results given to Maintenance Manager upon completion.( Approximately 35) All systems must be inspected semi-annual and results given to Maintenance Manager upon completion (3 hoods) Vendor is to provide repairs to all systems and components 24 hours per day, 7 days a week. Vendor must have local stock of parts Vendor must be prepared to correct all problems found during inspections at the request of the Maintenance Manager. Vendor must respond to emergency calls within 2 hours of request and same day for all non-emergency needs. Systems must be maintained in accordance with RI state fire codes and NFPA standards and codes. Inspections and testing reports shall be presented in person to the Job Corp's Maintenance Manager at time of inspection. One report is required for each master box. Inspections will be by appointment only with the Job Corp's Maintenance Manager. A copy of each test certification shall be forwarded to the local authority having jurisdiction within 10 days of testing completion. Vendor shall hold a Rhode Island license and all other licenses pertaining to above described work. Licenses may be subject to review upon request. In the event of an emergency, the Vendor will consult with the onsite Maintenance Manager or F&A Director to agree on estimated time and materials. The vendor shall obtain and maintain insurance as follows: •· Workers Compensation •· Comprehensive General Liability with Exeter Job Corps Center listed as additional insured. ($1,000,000 per occurrence). •· Comprehensive Automobile Liability ($1,000,000/combined single limit) •· Fire Legal Liability ($500,000 per occurrence) •· The proposed shall cover all expenses not covered by said insurance including all deductibles. Normal agency site hours are 8:00 am to 5:00 pm Monday thru Friday. Evaluation Criteria Evaluation Criteria Point Value Price: Responsible Contractor who can perform effective services at the lowest reasonable price. 60 Quality: Demonstrated experience and knowledge capabilities; Technical Excellence; Management Capabilities; Personnel qualification; Quality of the Tender packages; Technical excellence including where necessary, technical drawings, blue prints. 20 Past Performance : Past services to center or list of references for verification purposes. 10 Schedule: Explanation on how services will be carried out and the delivery timeframe. 10 TOTAL POINTS 100 Award will be made to the offeror who provides Best Value to the Center when all Evaluation Criteria are considered. Lowest Price does not guarantee award of Sub Contract. Adams and Associates retains the right to reject any and all bids.
