Description
Wage Determination (WD) Performance Work Statement (PWS) RFQ This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subparts 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. This is 100% set aside for a Small Business. The solicitation number is F1Q3MS5238A002 Group Exercise Instruction and this solicitation is being issued as a Request for Quotation. The Government intends to award a Firm-Fixed Price Contract. The solicitation document, incorporated provisions, and clauses are those in effect through the Federal Acquisition Circular 2005-83 and DFARS Publication Notice 20150930. The North American Industry Classification System Code (NAICS) is 611620 and the size standard is $7.5M. The contractor shall provide the following services: Group Exercise Instruction in accordance with the Performance Work Statement (PWS). The period of performance is 1 Nov 2015 - 30 Sep 2016. Attachment 1: Performance Work Statement Attachment 2: Past Performance Survey Attachment 3: Wage Determination Contractors are encouraged to offer their most advantageous/best quote. The contractor shall submit their quote by 16 Oct 2015, 3:00 p.m. (EST) to the contracting office by email in order to be considered. Quote is on an all or none basis and must be valid through 15 Dec 2015. All questions must be submitted by 13 Oct 2015 at 3:00 p.m. EST. The government will not accommodate any questions after 13 Oct 2015 at 3:00 p.m. EST. The following provisions and clauses apply to this acquisition and can be viewed through internet access at the Air Force FAR Site, http://farsite.hill.af.mil in accordance with 52.252-1, Solicitation Provisions Incorporate by Reference and 52.252-2, Clauses Incorporated by Reference. The provisions at 52.212-1, Instruction to Offerors-Commercial Items. Addendum to 52.212-1: Quote Preparation Instructions A. To assure timely and equitable evaluation of the quote, the offeror must follow the instructions contained herein. The quote must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The quote shall consist of three separate parts; Part I - Price, Part II - Technical (technical meaning proof of certifications reflecting the capability that the contractor is able to meet the requirements in the PWS) and Part III - Past Performance. B. The contractor agrees to hold the prices in its quote firm for at least 60 days from the date specified for receipt of quote. C. The contractor shall include the provisions at 52.212-3, Representation and Certifications-Commercial Items. And shall be registered in System for Award Management (SAM) Registration: https://www.sam.gov/portal/public/SAM/ End of Addendum Evaluation Factors: Lowest Price Technically Acceptable (LPTA). PART I - PRICE Please submit price quote in the following format: Line Item Description Qty Unit Price Per Unit Extended Price 0001 Group Exercise Instruction in accordance with the Performance Work Statement (PWS) 856 Each The extended amount must equal the unit price multiplied by the quantity. PART II - TECHNICAL CAPABILITY The technical approach shall, at a minimum, be prepared in a format consistent with the evaluation criteria for award set forth in the addendum to FAR clause 52.212-2, of this solicitation. The Technical qualification shall be a stand-alone document containing all information necessary for the Government to perform a complete technical evaluation. •A. The Offeror shall provide sufficient detail (Capability Statement) to substantiate the validity of all stated assertions. To facilitate evaluation, the information provided should be specific, detailed, and clearly demonstrate that the Offeror has a thorough understanding of the requirement for the accomplishment of this offer. •B. Offerors shall provide Instructor certifications within their quote from one of the following Air Force accepted certifying organizations: •· American Aerobic Association International/International Sports Medicine Assoc. (AAAI/ISMA) •· Aerobic and Fitness Association of America (AFAA) •· American Council on Exercise (ACE) •· American College of Sports Medicine (ACSM) •· American Fitness Professionals and Associates (AFPA) •· Cooper Institute (CI) •· National Association for Fitness Certification (NAFC) •· National Aerobics & Fitness Trainer Association (NAFTA) •· National Exercise Trainers Association (NETA) •· Professional Fitness Instructor Training (PFIT) •· World Instructor Training Schools (WITS) In addition, copies of cardiopulmonary resuscitation (CPR) certification must be provided for each instructor from one of the following organizations: •· American Heart Association •· American Safety and Health Institute •· Red Cross PART III - PAST PERFORMANCE : The contractor shall provide a list of no more than five (5) of the most recent and relevant contracts performed for Government agencies and commercial customers demonstrating that they have performed similar work in the past (within the last 3 years). The Past Performance will have the following information: •1. Contractor's Name •2. Contractor's point of Contact ( email & phone number) •3. Project Title •4. Detailed Description of Work ( Performed by the offeror) •5. Contract Number •6. Contract Dollar Value •7. Period of Performance •8. Name of Contracting Officer or POC including information (email & phone number) GENERAL QUOTE INFORMATION Offerors are hereby notified that initial quotes which are found incomplete may be rejected without affording the offeror an opportunity to satisfy the requirement of the Government. Notwithstanding the information included in support of the offeror's technical capability, the contractor must meet all requirements of this request for quote. The provision at 52.212-2, Evaluation - Commercial Items. The Government will award a Firm Fixed Price contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government. BASIS FOR CONTRACT AWARD: 1. This is a competitive selection in which competing offerors will be evaluated on Lowest Price Technically Acceptable (LPTA). Evaluation procedures shall be IAW FAR 15.101-1 when best value is expected to result from selection of the technically acceptable quote with the lowest price and acceptable past performance. The Government expects to award one firm-fixed price requirements contract as a result of this solicitation. The Government reserves the right to award without discussions or make no award should no vendor prove to be an acceptable offeror based on the criteria set forth in the solicitation. The US Government will make an award to the responsible (IAW the Federal Acquisition Regulation [FAR]) contractor whose offer conforms to the solicitation and are determined to be the most advantageous to the Government considering price and non-price factors. The Government will evaluate Part II, Technical Specifications on an Acceptable/Unacceptable basis. After an offeror has received an overall acceptable rating on the Technical, the Government will then evaluate the offeror's past performance. Offerors receiving an unacceptable rating for the Technical and /or on Past Performance will not receive further consideration. The solicitation requirements include all stated terms, conditions, representations, certifications, and other information required by 52.212-1 of this solicitation. The Government will evaluate Part III, Past Performance on an acceptable/unacceptable basis shown in the table below. The contracting officer shall seek recent and relevant performance information on all offerors based on (1) the past efforts provided by the offeror and (2) data independently obtained from other Government and commercial sources. Relevant performance includes performance of efforts involving Group Exercise Instruction that involved similar scope and magnitude of effort and complexities this solicitation requires. The purpose of the past performance evaluation is to allow the government to assess the offeror's probability of meeting the solicitation requirements based on the offeror's demonstrated past performance. The assessment process will result in an overall acceptable performance rating. Past performance regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement will be rated as highly as past performance information for the principal offeror. Offerors with no recent/relevant performance history or if the offeror's performance record is so sparse that no meaningful confidence rating can be reasonably assigned shall receive the rating "Unknown Past Performance," meaning the rating is treated neither favorable nor unfavorable and will be rated as acceptable. In evaluating past performance, the Government reserves the right to give greater consideration to information on those contracts deemed most relevant to the effort described in this Request for Quotation (RFQ.) If the lowest priced evaluated technically acceptable offer is judged to have an acceptable performance rating, that offer represents the best value for the government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other offers. If the lowest priced offeror is not judged to have an acceptable rating the next lowest priced offeror will be evaluated and the process will continue (in order by price) until an offeror is judged to have an acceptable performance or until all offerors are evaluated. The Government shall then make an integrated assessment best value award decision. Offerors are cautioned to submit sufficient information instructed above. Offeror's may be asked to clarify certain aspects of their quote ( for example, the relevance of past performance information) or respond to adverse past performance information to which the offeror has not previously had an opportunity to respond. Adverse past performance is defined as past performance information that supports an unacceptable rating on any evaluation element or any unfavorable comments received from sources without a formal rating system. Communication conducted to resolve minor or clerical errors will not constitute discussions and the contracting officer reserves the right to award a contract without the opportunity for quote revision. The government intends to award a contract without discussions with respective offerors. The government, however, reserves the right to conduct discussions if deemed in its best interest. 2. REJECTION OF UNREALISTIC OFFERS: Any quote which is unrealistic will be judged either as exhibiting a lack of competence or failure to comprehend the Government's requirements and may be so evaluated and rated, or rejected for such reasons. Furthermore, any significant inconsistency between the Technical qualification and Price, if unexplained, may be grounds for rejection of the quote. The Government will evaluate each quote strictly IAW its content and will not assume that performance will include areas not specified in the Offeror's quote. Nothing in this paragraph is to be construed as a promise by the Government to conduct a price realism analysis where it would otherwise not be required to do so. The decision to conduct or not to conduct such an analysis remains in the sound discretion of the Contracting Officer. PRICE: The Government will evaluate offers based on prices proposed for all CLINs and any other price related factors required by the solicitation. The offeror's quote will be evaluated for Reasonableness and Total Evaluated Price. 1. Reasonableness: The existence of adequate price competition is expected to support a determination of reasonableness. Price analysis techniques may be used to further validate price reasonableness. If adequate price competition is not obtained or if price reasonableness cannot be determined using price analysis, other techniques will be used. 2. Total Evaluated Price (TEP): The Government will utilize the following methodology to determine the TEP of each quote. The TEP will be calculated for evaluation purposes only. The TEP will be calculated as follows: i. CLINs will be calculated by multiplying the proposed unit price by the corresponding quantity. ii. The sum of all the CLINs evaluated prices will represent the total evaluated price iii. Award will be made to the responsible offeror that represents the best value to the US Government. TECHNICAL CAPABILITY: The following technical capability will be evaluated on an acceptable/unacceptable basis. If paragraph I through III are not submitted with the quote, the entire quote shall be considered unacceptable and shall be removed from consideration. The contractor at a minimum shall present the understanding and show how to accomplish the following tasks mentioned in the PWS: Experience providing beginner, intermediate, and advance low impact/high intensity floor aerobics. Experience providing kickboxing, indoor cycling, pilates, and yoga. Certifications from Air Force accepted certifying organizations. The following ratings will be used in rating technical capability: Acceptable: Proposed clearly meets the minimum requirements of the solicitation Unacceptable: Proposed does not clearly meet the minimum requirements of the solicitation PAST PERFORMANCE : Recent and relevant past performance is defined as past performance having occurred in the last 3 years preceding the date of the solicitation issuance. Relevant past performance is defined as past performance providing candidates for services similar in scope to this requirement and complexity. The Government will consider relevancy, recency, extent of the past performance and the size and complexity of the past performance relative to this solicitation's requirement submitted by offerors as well as any information gathered by the Contracting Officer. Offerors that have no relevant past performance history will be assigned an unknown past performance evaluation rating; unknown will be rated as acceptable. The following ratings will be used in rating past performance: Acceptable Based on the offeror's performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror's performance record is unknown. Unacceptable Based on the offeror's performance record, the Government has no reasonable expectation that the offeror will be able to successfully perform the required effort. End of Addendum
