Description
DR-4240-CA Urgent Haul & Install Statement of Work THIS IS A SEPARATE REQUIREMENT FROM THE MAIN HAUL & INSTALL CONTRACT CURRENTLY BEING SOLICITED HERE UNDER RFP # HSFE09-16-R-0001. *If you were to receive a contract for this urgent requirement, it does not give your company any priority or preference of award under solicitation HSFE09-16-R-0001.* FEMA has an urgent requirement to haul and install seven (7) Manufactured Housing Units (MHUs) on private sites: •2 installations in Lake County •5 installation in Calaveras County In order to protect the privacy and identity of the occupants, the addresses will only be released to the successful offeror. The government intends to award either a single purchase order for both counties, or two separate purchase orders; one for each county. Please identify if your quote is for Lake County only, Calaveras County only, or both counties. This is being solicited under Simplified Acquisition Procedures (FAR Part 13). Due to the urgency of this requirement this is an unrestricted solicitation (i.e., no small business or local set-aside); however, proximity to Lake and Calaveras Counties will be taken into consideration for award. It is anticipated that award will be made within 24-hours of this solicitation closing. The acquisition will be awarded on a best value basis, price and other factors considered. Submission Requirements and Evaluation Minimum Requirements The Government will award a contract resulting from this solicitation to the responsible offeror whose quote conforming to the solicitation provides the best value to the government. Factors to be considered are technical approach, how quickly the offeror can complete the required services, company experience and past performance. The offeror’s quote shall not exceed 5 pages and shall address: •A draft Phase-In Plan that addresses how you plan to prepare to assume the responsibilities of the Statement of Work. •Your Quality Control Plan (QCP) to be used in the performance of the contract. Specifically, address the quality control methods to be used and if they have been used successfully in other work performed. •The experience of the Offeror and of proposed subcontractors and/or partners intended to occupy a major role will be considered in evaluation and selection. •The degree to which the Offerors supply examples of accomplishments to indicate an ability to implement the proposed methods and techniques for solving problems will be assessed. Pages shall be numbered. Front matter does not count against the page limitation. The typewritten or printed letters shall be no smaller than 10 point or equivalent as the minimum size standard. Offerors shall ensure that the print is easily readable. Each page shall have not less than one-inch margins on each side of the page. Header/footer information (which does not include any information to be evaluated) may be included in the 1’ margin space. Each 8 and 1/2 by 11 inch “sheet” shall count as one page. FEMA will only accept electronic quotes sent via email to lilian.hutchinson@fema.dhs.gov. Submission Deadline is strict and no late submissions will be accepted. Offerors are encouraged to submit their quotes as early as possible. Alternate Information is not solicited, not desired, and will not be evaluated.
