- PRESCOTT VA HEALTH CARE SYSTEM Contract SUPPLEMENT B STATEMENT OF WORK CONTENTS I.Objectives II.Statement of Work/Requirements III.Special and Technical Requirements IV.Breakdown of Equipment/Sites to be tested I.Objectives: To issue a one year contract with 4 one year options to provide the Medical Gas annual inspections, testing and certification as stated in this Statement of Work. The objective of this contract is to provide for the annual inspection, testing and certification of the Medical Gas Systems. Also, annual requirements for Alarm testing and certification are included. II. Statement of Work: This service contract consists of two elements, which are: Element 1 - Contractor shall perform annual inspection, testing and certifying of the NAVAHCS medical gas system. Element 2 - Contractor shall perform annual Master and Area alarm testing and certification. Testing and certification of the Medical Center's medical gas systems shall comply with Engineering Policy Memorandum No. FM-20. This policy is used to schedule, monitor and document the testing and maintenance of the Medical Center's medical gas systems. These systems include medical air, oxygen, and medical vacuum. All critical components of the piped medical gas system including master signal panels, area alarms, automatic pressure switches, shutoff valves, flexible connectors and outlets, shall be tested, inspected and maintained in compliance with NFPA standards by the Contractor. Contractor shall have completed NFPA 99C Medical Gas Certifier training and provide a current certificate of qualification. Element 1 - Annually: The Contractor shall inspect, test and certify 100 percent of the total number of the medical gas outlets. Selection of the outlets will be in random locations, allowing for an overall view of the system during each quarterly maintenance cycle. All outlets will be tested and inspected during a ten-quarter period. The protocol for outlet testing and inspection is as follows. a.The Contractor shall check flow rate at each outlet. Oxygen, nitrous oxide, and medical air outlets must deliver a minimum of 100 liters per minute (lpm). Medical vacuum outlets must deliver a minimum of 85 lpm. b.Each outlet (except medical vacuum) shall be tested by the Contractor for suspended particulate by passing a gas stream through a particulate filter at maximum flow rate for five seconds. The presence of any discoloration or visible particulate matter will be documented and reported to the Maintenance Supervisor. c.Using a certified and documented meter, the Contractor shall test each oxygen, and medical air outlet for purity. Oxygen shall be delivered at a purity level greater than 99.0%. d.The Contractor shall check the static pressure at each outlet. Oxygen, and medical air, must have static pressure value of between 50 and 55 psi. Medical vacuum be a minimum of 12" of Hg. e.Each medical air outlets will be tested by the Contractor to ensure that conformance to grade "N" standards is being met. Using certified equipment, medical air will be checked for carbon dioxide, sulfur, nitrous fumes, carbon monoxide, oil concentration, and dew point. All levels must meet the criteria defined by CGA G-7.1 ANSI grade "N" air. f.The Contractor shall check each outlet for leaks and visible damage. g.Annual test and certification results shall be electronically recorded and forwarded by the Contractor to the Engineering representative within fifteen (15) days of testing. Element 2 - Annually a.The Contractor shall test master alarm panels and area alarms annually for proper operation. b.Alarm set points on the oxygen bulk tanks shall be verified annually by the Contractor. c.Zone, riser, source, and main line valves shall be inspected for proper labeling, locking devices, and visible damage annually by the Contractor. d.Carbon dioxide and dew point meters shall be calibrated annually by the Contractor. Using certified and documented equipment, each medical air compressor will be checked for carbon dioxide, sulfur, nitrous fumes, carbon monoxide, oil concentration, and dew point. All levels must meet the criteria defined by CGA G-7.1/ANSI for grade "N" air. e. Annual test and certification results shall be electronically recorded and forwarded to the Engineering designated authority within fifteen (15) days of testing by the contractor. General Notes: Notification of site visit by the Contractor for inspection, testing and certification shall be to the Engineering designated authority one (1) week prior to site visit. Testing, inspection and certification shall be accomplished by certified inspectors. Certificates shall be provided upon arrival prior to the certification process. Contractor to provide inspection, testing and certifying costs for one (1) year and four (4) additional years for each element of annual testing and certification of the Medical Gas and Master Alarm Systems at the Prescott Medical Center. The Contractor shall provide an annual review of the NAVAHCS Medical Gas System. The Contractor shall respond to requests for emergency service in the following manner: a.A competent service representative must respond and arrive at the Prescott VA Health Care facility within two hours from the time of request for service. b.All requests for repair service shall be considered emergency requests unless a statement to the contrary from the COR, POC or designee. c.The Contractor shall restore the facility to full performance within 24 hours of the originating call. B. Hazardous Material Assessment: No hazardous materials are expected to be encountered in the course of the project. If the presence of hazardous materials is suspected, notify the Contracting Officer's Representative immediately for assistance. C. Underground Utility Locations: Locating underground utilities is not required for this contract. D. Commissioning: No formal commissioning process is required. E. Phasing and Impacts: No phasing and impact is required. F. Interior Design No interior design work is required for this contract. G. New Construction Room Numbers: No rooms will be renumbered during this contract. H. C & A Requirements The C&A requirements do not apply, Security Accreditation Package is not required. Due to the non-technical nature of this requirement, there is no information to be protected. Project does not involve the storage, generating, transmitting or exchanging of VA sensitive information. No sensitive information will be handled by contractor performing work. III. SPECIAL & TECHNICAL REQUIREMENTS The technical requirements are specified in the Statement of Work. IV.
