WAIVER OF CLAIMS WORKMEN COMP AUTHORITY TO COMMIT W9 VENDOR QUESTIONNAIRE AT2015-127 CATERING SERVICES ON SITE FOR INDYPENDENCE JOB CORPS CENTER This is a Procurement Opportunity with Adams and Associates at the Indypendence Job Corps Center and not with the Federal Government. Edinburgh Street Edinburgh, Indiana 46124 Site Visit: October 22rd at 1:30 pm eastern time (Mandatory walk-through to bid) Proposal Due Date: October 29th, 2015, 3pm eastern time Submission of Proposals: Atterbury Job Corps Center Attn: Karla Chester, Purchasing Agent Purchasing.atterbury@adamsaai.com Fax: 812-314-6290 Project Summary: ON-SITE CATERING SERVICES – Indypendence Job Corps Center Adams and Associates, Inc., contracting for the U.S. Department of Labor for the operation of the IndyPendence Job Corps Center, a 100 student non-residential training facility located at 222 E. Ohio Street, Indianapolis, is currently soliciting written bids for on-site Catering Services at IndyPendence. The Atterbury Job Corps Satellite center, IndyPendence Job Corps Center, consists of 100 non-residential female students and has a requirement for providing breakfast and lunch on center. The Center seeks to receive proposals from food service operators who have demonstrated potential for providing food service programs that maintain high standards of service and quality food products. The vendor will provide food services for students, staff and visitors. Proposals should be thorough and provide detailed monthly menus so we may properly evaluate each Proposer’s ability to provide the specified services. All quotes should be submitted in writing to the Purchasing Department by October 29th at 3:00 pm eastern time. Quotes received after the deadline and/or without all requested documentation will not be considered. A mandatory pre-bid meeting will be held October 22nd at 1:30 pm Eastern Time. No one will be allowed on Center after that time. The walk-through will be held at IndyPendence Job Corps Center, 222 E. Ohio St., Indianapolis Indiana. Bidder must be present for walk through for bid to be considered. All questions regarding the Invitation to bid and/or scope of work must be submitted in writing to: purchasing.atterbury@adamsaai.com. No phone calls please. Period of Performance effective 12/1/15-11/30/16. II. BID REQUIREMENTS 1. Each bidder shall submit a bid based upon Part IV - Extent of Work that includes reference to the project listed in this scope of work. Any variations from the base scope of work are to be noted as distinctly separate from the base bid. 2. Bids shall be submitted for the work as a basis for the contract. Also must submit a bid for all option periods if options are included in the Scope of Work. 3. The contractor must include with his/her proposal, certification that he/she will conform to the latest Wage Rate. All wages are to be paid per the Service Contract Act for services over $2500 (Part IV - Extent of Work, page 6). 4. A minimum 90-day bid guarantee is required. 5. This project is tax-exempt. 6. Vendor shall obtain and maintain required insurance, in amounts to cover risk as stated below:  Comprehensive Automobile Liability Insurance ( $1,000,000 combined single limit)  Workers Compensation  Comprehensive General Liability Insurance ($1,000,000 per occurrence) with Atterbury Job Corps listed as additional insured.  Fire Legal Liability of at least ($500,000 per occurrence)  The proposed shall cover any and all expenses not covered by said insurance including all deductibles. 7. Final Bid Proposal shall be typed and submitted on official Company letterhead and must include the following:  Scope of Work ID#, Project Description, Project Specific Requirements, Hourly Cost  References if no past experience with Center  Copies of all Insurance Certificates  Copy of all licenses and certifications relevant to this proposal  Completed Vendor Questionnaire (with DUNS# if available) and W9  Small Business Certification, if any 8. Net 30 payment terms III. EVALUATION CRITERIA Evaluation Criteria Point Value Price: Responsible Contractor who can perform effective services at the lowest reasonable price. 60 Quality: Demonstrated experience and knowledge capabilities; Technical Excellence; Management Capabilities; Personnel qualification; Quality of the Tender packages; Technical excellence including where necessary, technical drawings, blue prints. 20 Past Performance: Past services to center or list of references for verification purposes. 10 Schedule: Explanation on how services will be carried out and the delivery timeframe. 10 TOTAL POINTS 100 Award will be made to the offeror who provides Best Value to the Center when all Evaluation Criteria are considered. Lowest Price does not guarantee award of Sub Contract. Adams and Associates retains the right to reject any and all bids. IV. EXTENT OF WORK SCOPE OF WORK ID #: AT2015-127 CATERING SERVICES-ON SITE Vendor is to return written bid on vendor letterhead, vendor paperwork, and all licenses for doing business in the State of Indiana, all employee licenses required for performing work in the state of Indiana, Certificate of Liability Insurance and list of references if no past experience with Center. Vendor must include the Scope of Work Identification number on submitted bid. The period of performance is effective 12/1/2015 – 11/30/2016. Overall low bid does not constitute bid award. Atterbury Job Corps Center is a federally funded facility and entitled to GSA pricing. Payment terms are Net 30 days from date of invoice. Vendor is required to show number of meals served on invoice and provide certified payroll records with invoice. All wages are to be paid per the Service Contract Act for services over $2500. For service contracts over $2500 contractors are required to pay the federal minimum wage as provided in Section 6(a)(1) of the Fair Labor Standards Act. Vendor must provide letter stating they will conform to Service Contract Act Wages. Services to be provided: All meals will be prepared off site and served on-site in the designated area at 222 E. Ohio Street, Indianapolis, Indiana. Vendor shall hold all licenses necessary to do business in the state of Indiana and shall provide copies of all licenses necessary for preparing and serving food in the state of Indiana to include a comprehensive working HACCP plan. The bid proposal will have a breakdown of meal cost and a line item proposal for each item listed below. The contractor shall provide for the delivery and operation of food services, including: 1. All personnel 2. Uniforms and required safety equipment for personnel 3. All food supplies 4. All cleaning supplies and cleaning services 5. All non-food supplies 6. All equipment that is not currently available at the facility 7. The Center will provide the dining facility. The contractor shall operate the IndyPendence Job Corps Center Food Services to provide services as follows: a) Two meals per day consisting of breakfast and lunch except on holidays and weekends, when no meals are necessary. No more than 5 hours shall elapse between meal services. b) Meals that meet daily nutritional guidelines and include at a minimum: 1. Choice of two entrees at lunch of protein foods 2. Two and one-half servings of vegetables and/or fruits daily 3. Two servings of grain products 4. Two servings of milk 5. One vitamin C foods and sources of iron daily, and one vitamin A food every other day c) A minimum of a 28-day cycle catering menu d) Meals and food items that meet the dietary needs and desires of a diverse population, including ethnic, vegetarian, low-fat alternatives, etc e) Meal service to accommodate students with special needs, i.e. food allergies f) Ensure that food handling practices meet state, local or US Public Health Service Food Codes (whichever is more stringent). All meat products shall meet US Department of Agriculture standards g) Operate serving and meal preparation areas that are clean, attractive, and comply with state or local (whichever is more stringent) food handling and sanitation standards for the food service industry. Maintain the cleanliness of all areas inside and immediately surrounding area outside of the dining facility including floors, ceilings, walls, tables, chairs, dining areas h) Post the most recent environmental health inspection report at the serving line entrance i) Ensure that the staff complies with state or local food handling requirement j) Develop procedures to obtain and address student input into the planning and evaluation of the content and quality of the meals and service provided SERVICES PROVIDED BY THE CENTER Center shall provide: a) A staff member to serve as liaison between Center and Provider and be responsible for assigning students to the program b) All utility service: Water, Sewer, Electricity c) Providing a staff supervisor during meal service periods TO: Participants The following documents are to be returned with your bid package: • Bid proposal on vendor letterhead • Vendor questionnaire • W9 • Authority to Commit • Waiver of Workmen’s Comp • Debarment Letter • Certificate of Liability Insurance • Statement of Compliance (Service Contract Act) on vendor letterhead Comprehensive HACCP Plan Any bid submitted without the requested documents will not be considered. Bids must be received by 3 pm eastern time on October 29th, 2015.
