Description

MTC Terms & Conditions W-9 Self-Certification Form Exhibit A Bid-Sheet Request for Quote This is a subcontracting opportunity with Management and Training Corporation (MTC), operator of the Tongue Point Job Corps Center, is requesting proposals for Various Building Supplies of an indefinite quantity which will be ordered throughout the next 10-12 months. Details of the solicitation are in the attached request for quote (RFQ# TPJCCBuildingSupplies15-28.pdf). All bids must be completed on the attached Exhibit A Bid sheet to be considered valid. Please note that this is a repeat request for quote in reference to RFQ: TPJCC Building Supplies 15-22. We are re-posting in order to obtain quotes from a larger pool of bidders. In addition to the bid, all valid bids must include a W-9 and a completed MTC vendor self-certification form (attached). Bidders must self-certify under the listed NAICS code (NAICS codes can be found at http://www.census.gov/eos/www/naics/): This solicitation is listed under NAICS code: 444130 Hardware Stores All bids are due Thursday October 22, 2015 by 5:00 PM (PST).
