Description
SOW RFQ W25G1V-16-T-0017 Statement of Work for Relocation of Mobile Work Platform I. SCOPE Provide the required supervision, personnel, tooling and equipment to move mobile work platform from test area on Powder Smoke Ridge to new test area near Building 69 at Tobyhanna Army Depot. The mobile work platform was originally purchased and installed in 2012. The platform was manufactured by Steele Solutions, Inc. While the platform is mobile and may be moved around on a concrete pad, the casters are not designed for over the road transportation of the platform. The platform is approximately 18' x 15' x 24' high and weighs approximately 10,000 pounds. The relocation shall include but not be limited to the following tasks as a minimum: A. Disassemble platform as needed for transport from area near Building 49, B. Transport to area designated near Building 69 (it may take multiple trips to transport all the materials & components) (this is a distance of around 2 miles between Bldg 49 and Bldg 69). C. Re-assemble and return platform to serviceable condition as it existed prior to relocation. All work shall be performed during normal business hours, Monday through Friday, 7:30 AM - 4:00 PM., unless coordinated with depot point of contact. Except for Federal Holidays or days TYAD is closed for any purpose. II. WARRANTY The work accomplished under this specification shall be warranted for a period of 1 year from the date of final acceptance. Failures resulting from faulty parts not associated with or provided under this contract are not required to be warranted. Contractor shall follow all TYAD safety and environmental rules and all other TYAD rules regarding proper conduct (see specific requirements below). III.. RESPONSIBILITIES A. Contractor The Contractor's responsibilities shall include but not be limited to the following: 1. Project Management & on-site supervision 2. Coordination with Tobyhanna Army Depot's (TYAD's) Point of Contact (POC) regarding the relocation. 3. On-site safety program 4. Provide a Project Schedule within 10 days of award via hard copy, email or electronic media 5. Provide for Quality Control for both material and workmanship 6. Erect/provide safety barriers and precautions around work area as needed 7. Provide necessary tools, equipment, labor and supervision to complete relocation. 8. Neatness and organization of the work site, free of debris accumulation (daily) 9. Providing all required material handling equipment/lifting equipment to relocate mobile platform in a safe manner (a government flatbed may be used to transport platform material from one location to the other but it shall be the contractors responsibility to load the trailer, strap material down for transport, unstrap material and unload trailer). 10. The Contractor shall specify any special utility requirements not currently existing in the Bldg 49 area or the Bldg 69 area if necessary for the relocation. B. Government Shall be responsible for: 1. Providing clear area around current work platform location to allow for platform disassembly and clear area at new location for platform re-assembly. 2. Identify a Point of Contact (POC) to serve as a single point of contact to coordinate contractor's efforts with depot operations and personnel. 3. The government will provide a flatbed trailer and driver for transporting platform material from current location to new location and tow trailer from one location to the other. Vendor will load and unload at each location. IV. PERIOD OF PERFORMANCE/ INSTALLATION a.) TYAD's POC to be notified 10 day prior to commencement of on-site work. b.) All work and requirements in this SOW shall be completed no later than 70 days from the date of award V.. SUBMITTALS The government shall have 10 days to review and approve all submittals. Submittals provided to the COR (via hard copy, email, or electronic media) shall include: a.) Schedule of work (within 10 days of award) b.) On-site safety program c.) Utilities required for the operation of contractor's equipment if other than standard requirements. d.) Contractor Quality Assurance Plan (within 30 days after award) VI. Final Acceptance/ Payment Acceptance will consist verification by government Point of Contact (POC) or Contracting Officer Representative (COR) that platform has been relocated and re-assembled and is back in its serviceable condition prior to relocation.
