STATEMENT OF EQUIVALENT RATES FOR FEDERAL HIRES STATEMENT OF WORK PRICING SCHEDULE WAGE DETERMINATION 2005-2140 REV. 17 CERTIFICATION REGARDING RESPONSILITY DELINQUENT TAX REPORT CDRL A002 (ACCIDENT/INCIDENT REPORT) CDRL A001 (SAFETY & HEALTH PLAN) INDUSTRIAL SAFETY AND HEALTH REQUIREMENTS This is a combined synopsis/solicitation for commercial items prepared IAW the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested, and a separate written solicitation will not be issued. This solicitation number F3QTAK5190A002 is a Request for Quote. Cert and Rep must be returned with quote. This is not a SB set-aside. Prior to award of any DOD contracts, offerors must be registered in System for Award Management (SAM) and all Representations and Certifications must be completed. BUSINESS SIZE STANDARD: (a) NAICS: 711219 (b) DOLLARS: $11M This requirement is for INTRAMURAL SPORTS OFFICIALS AT ROBINS AFB, GA. PERIOD OF PERFORMANCE: 29 OCT 2015 - 28 OCT 2016 Price(s) should be FOB Destination (Robins AFB GA 31098). In addition to price and delivery, quote should include your cage code, DUNS number, and taxpayer ID number. Requested delivery date is 30 days ARO or earlier. THIS STATEMENT IS FOR INFORMATION ONLY; IT IS NOT A WAGE DETERMINATION Employee Class Monetary Wage-Fringe Benefits GS-0188-05 Recreation Specialist $16.46 1)Contributions of 5.1% of basic hourly rate for Health Insurance. 2) Contributions of 7% of basic hourly rate for retirement. 3) ten paid holidays. 4) Paid annual leave (vacation) of two hours each week for employees with less than three years of service, three hours each week for employees with three but less than fifteen years of service, and four hours each week for employees with fifteen or more years of service.
