- Burnham Data Sheet Wage Determination No. 2005-3017 Revision 16 This is a competitive Small Business Set Aside combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Offerors submitting quotations must respond in accordance with the instructions provided in this solicitation. Solicitation number AG-349B-S-16-6044 is being issued as a request for quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-84. This requirement is a competitive Small Business Set Aside under NAICS 238220, business size standard is 15.0 Million. Offerors are responsible for complying with the submission instructions contained in this combined synopsis/solicitation to include the submission of all information requested. Small Businesses with the demonstrated capability and financial capacity to provide required services and can meet the time for completion of 30 days from date of award are invited to respond. Submission of quotations shall be in accordance with the instructions identified under Item (A.1) listed below. Quotation must be submitted in writing, include all required information specified under Item A.1, and be received on or before 4:00 P.M. October 30, 2015 at the address specified under Item A.1. Quotations sent by facsimile or electronic mail are not acceptable, will be considered non-responsive, and will be rejected. BACKGROUND INFORMATION: The United States Department of Agriculture, National Center for Cool and Cold Water Aquaculture (USDA-NCCCWA) located at 11861 Leetown Road, Kearneysville, WV requires repair of two Model 4F-450A-50-0-GP, #2 fuel oil fired hot water Burnham boilers. All work shall be performed in accordance with manufacturer specifications and accepted commercial practices. DESCRIPTION/SPECIFICATIONS/AND WORK STATEMENT : The contractor shall provide all services including labor, materials, supplies, supervision, tools, equipment, insurance, and transportation necessary for maintenance of two boilers to include removing and properly disposing of existing tubes (110 tubes each) from two Model 4F-450A-50-0-GP, #2 fuel oil fired hot water Burnham boilers and replacing with new tubes. (46 tubes are approximately 90" in length and 64 tubes are approximately 71.5" in length per boiler). Replacement boiler tubes shall be in accordance with the following material specifications: material specification number SA178, Grade A, 2" diameter,.095" / 12 guage. Upon completion of each boiler the contractor shall perform a hydrostatic test to ensure there are no leaks. Hydrostatic tests shall be performed to 30 psi for 30 minutes at room temperature. There shall not be any allowable leakage at the tube sheets. A written report detailing the test shall be provide to the Contracting Officer Representative upon task completion. Work may only proceed on one boiler at a time to allow operation of the second boiler. Upon completion, a minimum one year commercial warranty is required to be provided to the Contractor Officer Representative for materials and installation services. All work shall be performed by trained and certified personnel and shall adhere with all local, state, and federal codes. Service Contract Act applies in accordance with attached Wage Determination No. 2005-3017, Revision 16. Contractors interested in visiting the work site must request a site visit by calling (304) 725-3451 x341 within sufficient time for submission of a quotation prior to closing date. A site visit is offered for the purpose of providing contractors with the opportunity to be familiar with the site which may be helpful in the preparation of quote. The site visit is not mandatory for quote submission, however, failure to visit the site will not relieve or mitigate the successful contractor responsibility and obligation to fully comply with the terms, conditions, and specifications contained and/or referenced in this document. Management/Quality Control: The Contractor shall manage the total work effort associated with the services provided to assure fully adequate and timely completion of these services. The contractor shall provide trained and certified technicians with the expertise to assure the performance of the work in accordance with sound and efficient commercial business practices. Work Schedule : Hours of operation are 7:00 am to 5:00 pm Monday through Friday with the exception of official federal holidays.
