Description
Statement of Work SF1449 DOL Wage Rates for Miami/Dade County Commercial Far Clauses This combined synopsis/solicitation for services is prepared in accordance with the format in subpart 12.6 of the Federal Acquisition Regulation (FAR) as supplemented with additional information included in this notice. This synopsis/solicitation is being advertised as Small Business Set-Aside only. This announcement constitutes the only solicitation; no separate written solicitation will be issued. The solicitation number is HSCG28-16-Q-7AA004. Applicable North American Industry Classification Standard (NAICS) codes are: 562998 MISC Waste Management Service Standard size in Millions ($7.0) This requirement is for a fixed price contract. The contract will be awarded using simplified acquisition procedures in accordance with FAR part 13. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate the proposals: 1. Technical Capability of the services offered to meet/exceed the Governments Requirements per the Attached Statement of Work. 2. Past Performance 3. Price of items offered to the Government Technical and past performance, when combined, is ___60%_______ When submitting your proposals, request a price breakdown of the following: a. Cost of Materials b. Cost of labor and supervision for the cleaning and testing of tanks per SOW Anticipated award date: 10 business days after close of solicitation, OOA 13 November 2015 Quotes are to be received no later than close of business (3 p.m.) on 03 November 2015. Telephone requests of quotes, will not be accepted, a formal notice of changes (if applicable) will be issued in FEDBIZOPS (www.fbo.gov). Wage determinations: 2005-2119, Rev 17 dated 07/08/2015 SCOPE OF WORK: Contractor to remove estimated 1800 gallons of JP-5 waster products remaining inside (3) 35,000 gallon above ground storage tanks and est. 500 gallons of petro waste from 6,000 gallon storage tank, pressure clean and dry (3) 35,000 gallon JP-5 storage tanks and other listed requirements per SOW. Geographical range: Price Range: NTE $93,500 Performance Period: Performance of work is expected to commence NLT 10 days after award is made. Extension period must be approved by the Contracting Officer for any unforeseen delays. The Contractor shall provide all labor, supervision and any other supplies, materials and equipment necessary to perform the services required.
