Description
Addendum (i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii)The solicitation number is HHS-NIH-NIDA-SSSA-CSS-2016-002 and the solicitation is issued as a Request for Quotation (RFQ) on a competitive basis containing a set-aside restriction for small business concerns. This acquisition is for a commercial item or service and is conducted under the authority of the Federal Acquisition Regulation (FAR) Part 13-Simplified Acquisition Procedures; FAR Subpart 13.5-Test Program for Certain Commercial Items; and FAR Part 12-Acquisition of Commercial Items, and is expected to exceed the simplified acquisition threshold. (iii)The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-84 effective October 10, 2015. This requirement is solicited as full and open with a socio-economic total small business set-aside restriction. The contract type is anticipated as fixed price level of effort pricing. (iv)The North American Industry Classification System (NAICS) code applicable to this procurement is 541990 - All Other Professional, Scientific and Technical Services with the associated business size standard 15.0 million Dollars. (v)Cardiovascular Testing Interpretation Clinical Contractor Support Services (vi)Background: The Intramural Research Program (IRP) in the National Institute of Aging (NIA) at the National Institutes of Health (NIH) is comprised of nine scientific laboratories, the Translational Gerontology Branch, and ten core facilities. The research program has three main focus areas: Neuroscience, Aging Biology and Translational Gerontology. The Laboratory of Clinical Investigation is located at Harbor Hospital in Baltimore, Maryland and contains the NIA Clinical Research Unit - home to the Baltimore Longitudinal Study of Aging. The Baltimore Longitudinal Study on Aging (BLSA) is America's longest-running scientific study of human aging since beginning in 1958. The main emphasis of the study has been to characterize how an individual ages across the adult life span. This cohort study has continually recruited subjects from its onset. Over the years, changes have occurred in research questions and the measurements derived from research participants. The purpose of this requirement is to acquire the services of an expert in clinical cardiovascular research who will be responsible for coding, interpreting and performing quality control of cardiovascular disease scientific data generated by the clinical research program. (vii)Statement of Work: Independently and not as an agent of the Government, the contractor shall furnish all the necessary services, qualified personnel, materials, equipment, and facilities, not otherwise provided by the Government as needed to perform the cardiovascular testing services described in this solicitation. The successful Contractor shall be a clinician trained in cardiovascular medicine to perform coding, quality control and interpretation of cardiovascular testing results performed at the NIA clinical center. This will include reading, analyzing and interpreting resting electrocardiograms, echocardiograms, treadmill stress tests, processed ambulatory Holter reports, and pulse wave velocity studies. The contractor will perform measurement oversight and interpretation services using the center's custom-built electronic reporting system, the BLSA-modified Minnesota coding system. Testing results of routine BLSA exams and selected intervention trials shall be recorded using the center's electronic reporting system, Xcelera. Specifically, the contractor shall perform coding, quality control, interpretation and data management as it relates to analyzing and recording the following BLSA test results using the center's electronic reporting system, Xcelera: * Resting electrocardiograms and Treadmill stress testing results with the BLSA-modified Minnesota coding system for ischemia, arrhythmia, and relevant electrocardiographic abnormalities; * Echocardiographic studies; * Carotid artery intimal medial thickness measurements; * Pulse Wave Velocity study measurements; * Ambulatory Holter reports The Contractor shall ensure that all data entered and recorded is efficiently imported into the BLSA study database. The Contractor will also be required to oversee the electronic cardiovascular test reporting system. This will include identifying and making recommendations to the Information Technology department on proper functioning matters of the system. Reporting requirements: The contractor is required to provide written progress reports on a weekly basis during the contract period of performance. Progress reports shall cover all work completed during the specified period and shall present the work to be accomplished in the subsequent period. This report shall also identify any problems that arose and a statement explaining how the problem was resolved. This report shall also identify any problems that have arisen but have not been completely resolved and provide an explanation as to potential resolution and the short- and long- term possible impact. Deliverables: Malpractice Insurance: The contractor must provide evidence of malpractice insurance within 15 days of contract award. The Contractor shall submit the following deliverables as required by the INFORMATION AND PHYSICAL ACCESS SECURITY article contained in the contract: 1.Roster of Employees Requiring Suitability Investigations; 2.Contractor-Employee Non-Disclosure Agreement(s). Information Security Training: Mandatory Training: Prior to performing any work on the contract, the Contractor must complete the following training: All Contractor employees having access to (1) Federal information or a Federal information system or (2) sensitive data/information as defined at HHSAR 304.1300(a)(4), shall complete the NIH Computer Security Awareness Training course at http://irtsectraining.nih.gov/ before performing any work under this contract. Contractor will not be able to access any federal information systems without successfully completing this training. Thereafter, Contractor employees having access to the information identified above shall complete an annual NIH-specified refresher course during the life of this contract. The Contractor shall also ensure subcontractor compliance with this training requirement. The Contractor shall maintain a listing by name and title of each Contractor or Subcontractor employee working on this contract and having access of the kind in the preceding paragraph, who has completed the NIH required training. Any additional security training completed by the Contractor/Subcontractor staff shall be included on this listing. The list shall be provided to the COR and/or Contracting Officer upon request. Role-based Training: HHS requires role-based training when responsibilities associated with a given role or position, could, upon execution, have the potential to adversely impact the security posture of one or more HHS systems. New designated staff must complete the appropriate role-based training requirement within three months of their initial start date. The Contractor shall provide the COR with the name, position title, e-mail address, and phone number of all new contract employees working under the contract and provide the name, position title and position sensitivity level held by the former incumbent. If an employee is filling a new position, the Contractor shall provide a position description and the Government will determine the appropriate position sensitivity level. The list shall be provided to the COR and/or Contracting Officer upon request. Rules of Behavior: The Contractor shall ensure that all employees, including subcontractor employees, comply with the NIH Information Technology General Rules of Behavior ( https://ocio.nih.gov/InfoSecurity/training/Pages/nihitrob.aspx ), which are contained in the NIH Information Security Awareness Training Course http://irtsectraining.nih.gov. The contractor shall comply with Section 508 of the Rehabilitation Act (29 U.S. C.§794d), as amended by the Workforce Investment Act of 1998 (P.L. 105-220), August 7, 1998. Government's responsibilities: The government will provide computers to the contractor configured for VPN access and equipped with interpretation applications (i.e. Echo interpretation requires Osirix on Mac-OS, Reporting requires SAS on Windows OS), study interpretation applications, and statistical software for use in the performance of this contract. Place of Performance: The work will be performed at Harbor Hospital, 3001 S. Hanover Street, Baltimore, MD 21225 and the BRC, 251 Bayview Blvd., Suite 100, Baltimore, MD 21224. The contractor may perform duties remotely using the center's electronic study reporting system. Period of Performance: The anticipated period of performance is one base year effective 12 months on or around November 4, 2015 through November 3, 2016 and two (2) additional subsequent 12-month periods of performance as Option Years. The Government estimates that 1,250 hours of work on an annual basis will be required to achieve this Statement of Work (approximately 24 hours per week). Contracting Officer's Representative: The Contracting Officer's Representative (COR) will represent the Government for the purpose of this contract and will be specified at time of award. The COR is responsible for: (1) monitoring the Contractor's technical progress, including the surveillance and assessment of performance and recommending to the Contracting Officer changes in requirements; (2) interpreting the statement of work and any other technical performance requirements; (3) performing technical evaluation as required; (4) performing technical inspections and acceptances required by this contract; and (5) assisting in the resolution of technical problems encountered during performance. The Government may unilaterally change it's COR designation at any time throughout the performance period of this contract. The COR will be determined at time of award. (viii)Information Security Program Requirements: Information security requirements is applicable to this acquisition and will be contained in any contract resulting from this solicitation. The provisions for INFORMATION AND PHYSICAL ACCESS SECURITY applicable to this acquisition is included as a separate attachment to this solicitation. Cost for all security requirements shall be incurred by the contractor. (ix)The provision at FAR clause 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition. (x)The provision at FAR clause 52.212-2, Evaluation - Commercial Items, applies to this acquisition. (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Technical and past performance when combined is significantly more important when compared to price. The following factors shall be used to evaluate offers: 1.Technical Approach, knowledge, and experience. Contractor shall clearly indicate prior knowledge of and experience with the preparation of longitudinal data into SAS databases suitable for analysis by mixed models based approaches. That Offeror shall include applicable Curriculum Vitae of key identified personnel. Offerors shall also submit documented evidence of published manuscripts in peer-reviewed journals of longitudinal studies of aging related cardiovascular changes. 2.Past Performance. The offeror must submit with their proposal a list of the last three (3) contracts completed during the past two (2) years and / or contracts awarded currently in process that are similar in nature to the work scope. Contracts listed may include those entered into by the Federal Government, agencies of state and local governments and commercial concerns. Newly formed entities without prior contracts should list contracts and subcontracts as required above for all key personnel. For each contract, subcontract, or reference include: (i) Name of Contracting Organization; (ii) Contract Number (for subcontracts, provide the prime contract number and the subcontract number); (iii) Contract Type; (iv) Total Contract Value; (v) Description of Requirement; (vi) Contracting Officer's Name and Telephone Number; (vii) Program Manager's Name and Telephone Number; and (viii) North American Industry Classification System (NAICS) Code. The offeror may provide information on problems encountered on the identified contracts and the corrective actions, if any. Offerors will be evaluated on its performance under existing and prior contracts for similar products or services. The Government is not required to contact all references provided by the offeror. Also, references other than those identified by the offeror may be contacted by the Government to obtain additional information that will be used in the evaluation of the offeror's past performance. 3.Price. Basic cost and price information in the Offeror's business proposal must contain sufficient information to allow the Government to perform a basic analysis of the reasonableness of the Offeror to supply the required services. Price quotations should be formatted to include: 1) Estimate of the number of hours required; 2) Hourly rate; 3) other direct costs or materials, if applicable; 4) total cost. To determine price reasonableness, Contractors should include with their offer a commercial item price list showing the hourly rate contained in the quotation. A commercial item price list is any publically-accessible article listing the units and prices offered for the same, or similar, type of work in the commercial marketplace. If no commercial item price list exists, contractor should provide a copy of a redacted previous invoice showing unit cost and total cost of the same, or similar, type of work performed for commercial or government entities. Alternatively, Contractors may submit with their offer, a redacted copy of a previous contract, order, or agreement executed by a commercial or public entity indicating the unit and total cost of the same, or similar, type of work performed. Responses to this solicitation must include clear and convincing evidence of the Offeror's capability of fulfilling the requirement as it relates to the technical evaluation criteria. Offerors may respond with a technical proposal indicating their ability to provide the technical specifications stated in this solicitation and an itemized price-cost proposal. Responses will be evaluated based on contractor's ability to meet the government's essential requirements stated in this notice and on ability to achieve the statement of work described in this solicitation. (xi)The provision at FAR Clause 52.212-3, Offeror Representations and Certifications Commercial Items, applies to this acquisition. (xii)The provision at FAR Clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. (xiii)The provision of FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies to this acquisition. (xiv)The provision at FAR clause 52.227-14, "Rights in Data-General" applies to this acquisition. The NIA shall have unlimited rights to and ownership of all deliverables including reports, recommendations, briefings, work plans and all other deliverables. The definition of "unlimited rights" is contained in Federal Acquisition Regulation (FAR) 27.401. (xv)There are no additional contract requirement(s) or terms and conditions applicable to this solicitation. (xvi)The Defense Priorities and Allocations System (DPAS) are not applicable to this requirement. (xvii)In addition the Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size must be included in the response. All Offerors must have an active registration in the System for Award Management (SAM) www.sam.gov." All responses must be received no later than October 30, 2015, 4:30 PM, Eastern Time, as specified in this announcement. Responses to this RFQ must not exceed a combined 25 pages in length, exclusive of appendices and Curriculum Vitae, and be submitted in Microsoft Word or searchable PDF format in a font size of 11 or 12 point and font type of Arial or Times New Romans. Margins must be 1-inch wide (top, bottom, left, and right). Responses must references this solicitation number and include the name of the contractor.
