- This is a sources sought announcement ONLY and is NOT a solicitation for proposals or quotes. The information gathered from this request for information (RFI) is for planning purposes only. The Government is not obligated, nor shall it pay for, any information received from potential sources as a result of this Sources Sought Notice. Should a solicitation be posted in the future, the acquisition will be processed under Simplified Acquisition Procedures, utilizing associated North American Industry Classification System (NAICS) Code 325120. Description of Requirement: The Gainesville & Lake City VA Medical Center (VAMC) has a requirement for a vendor to supply medical grade bulk oxygen to the medical center. The VA is seeking sources to provide all needed components to support a Food and Drug Administration (FDA) certified bulk oxygen main supply system for medical gas in accordance with National Fire Protection Agency (NFPA) 99. The need involves the rental of a primary tank for the Gainesville VAMC location with a capacity of 6050 gallons and rental of a reserve tank with a 1500 gallon capacity all new equipment. The Lake City VAMC currently owns one 3500 gallon primary and one 525 gallon reserve tank, which will need to be maintained along with filling. Interested vendors shall include all required components of the bulk oxygen supply, connection to VAMC's existing oxygen system, preventive maintenance, annual training, inventory management of the bulk systems, and all necessary documentation. The supply systems will be located at the existing locations at the VAMC in Gainesville, Florida and Lake City. The system shall be maintained too ensure uninterrupted supply of oxygen to the medical center. All work shall be done in accordance with NFPA, Center for Disease Control (CDC), Joint Commission for Accreditation Hospital Organization (JCAHO) and American Society of Heating, Refrigerating and Air-Conditioning Engineers (ASHRE) Guidelines. 1. Vendor must be FDA certified to produce medical grade liquid oxygen daily needs as well as in the event of an emergency situation, i.e. hurricanes, flooding, tornadoes, etc. - Documentation is required. 2.Must be able to monitor via telemetry monitoring system to manage levels of medical gas. 3. Vendor must be currently certified to perform and complete FDA annual inspection for bulk liquid oxygen system. 4. Vendor must have current trained and certified staff to perform any required maintenance and inspections at the local site or available within 2 hours of time of notification. 5. Vendor must be able to deliver Monday-Friday (with the exception federal or training holidays) between the hours of 0800 and 1530 for the Gainesville VAMC and Lake City VAMC. Vendor must be available to make emergency deliveries within 24 hour of notification as required or as mission dictates. 6. Vendor must provide packing list at time of installation/delivery to ensure proper payment. 7. Vendor must make all records available ON-LINE (web access) including delivery history, level history, reading history, and account information that is downloadable in spreadsheet format. 8. Must have an emergency contact system in place in the event of catastrophic or emergency situations. CONTRACTOR PERFORMANCE REQUIREMENTS: 1. Vendor shall supply contractor-owned equipment: All delivery, installation, labor, materials and pick up charges to be paid by the vendor. 2. The vendor shall maintain uninterrupted service to the medical center in the events of a failure of the primary tank or until the primary tank can be refilled. 3. A reinforced concrete pad which meets the minimum standards in the existing location. 4. A Pressure Control Manifold, all lines/piping, connections, required to connect tanks to manifold and to existing hospital piping at pad location and a Pressure Control Manifold, all lines/piping, connections, required to connect tanks to manifold and to existing hospital piping at pad location for reserve. 5. An off-site monitoring system (telemetry system), in which the successful vendor will be able to independently monitor the system and refill requirements. Provide all equipment required to fill tanks as needed. Provide Purity of Oxygen: 99.996% - Medical Grade (a.k.a. USP Grade) Frequency of Fill: As Needed Vendor shall own and maintain the tanks, manifold system, and all associated equipment from the tanks to the connection point at pad site. Vendor must provide emergency service plan documenting what services will be available in the event of complete loss of system, catastrophic event or emergencies. At the end of the contract, the contractor is responsible for removal of all tanks and equipment at no cost to the Government. 6. HOURS OF DELIVERY: Monday - Friday 0800 to 1530, as well as available to the site location within 2 hour of notification for Emergencies 24/7. ROUTINE SERVICE: The vendor will provide routine service Monday through Friday, 0800 to 1530 at the Gainesville VAMC. On-site service will be provided within (2) hours, excluding holidays, upon notification. PREVENTATIVE MAINTENANCE: The vendor shall provide preventive maintenance, emergency response, routine service, and inventory management for the Bulk oxygen system as required by NFPA 99, JCAHO, VA and other related codes/standards. Provide a complete written preventive maintenance report to the project official listing operation of each device as required by referenced codes. Provide documented annually operational training on site to associated personnel. During the scheduled inspections as required by referenced codes, the contractor will perform a thorough maintenance check of the system including but not limited to: Document training for safety plan and daily operations. Document Gas emergency plan JCAHO EC.1.4 Reserve supply change over system Vaporizers and automatic switch assemblies Document and inspect all tank and control valves. Check and document all main source alarms for both tanks. Check and document all gauges,Check and document operation of all pressures regulators. Inspect and remove icing from vaporizer as required. Provide a general inspection of all system components including wiring, contacts, air pressure switches, belt tension etc. Assigned VA staff will perform daily inspections of tank site and tank levels. Maintenance of site grounds is the responsibility of each VA facility. EQUIPMENT REPAIR not listed above will need prior approval from Contracting Officer before the execution of any additional work. PAYMENT SCHEDULE: Any potential contract would be billable on a monthly basis and all requirements must be satisfied before any monthly payments approved.
